體表電極代替多導食道電極測定慢性阻塞性肺疾病患者呼吸中樞驅動的可行性

【摘要】:目的探討體表電極代替多導食道電極測定慢性阻塞性肺疾病(慢阻肺)患者呼吸中樞驅動的可行性。方法研究對象為呼吸科門診29例穩(wěn)定期中重度慢阻肺患者,均為男性,平均年齡(65.6±7.2)歲。采用體表電極和多導食道電極同時記錄患者吸入支氣管舒張劑前后靜息狀態(tài)和等二氧化碳高通氣(MIV)時膈肌肌電和肺通氣量的變化,比較兩種方法測定的呼吸中樞驅動的差異。采用重復測量數(shù)據(jù)的方差分析,兩兩比較用最小LSD檢驗,相關分析采用Pearson檢驗。結果吸入支氣管舒張劑后,患者靜息和MIV時的肺通氣量增加,體表電極和多導食道電極測定的呼吸中樞驅動均同步降低,呈顯著相關(r=0.660,P0.01)。結論采取措施盡可能減少體表電極干擾的情況下,體表電極可以代替多導食道電極測定慢阻肺患者的呼吸中樞驅動。
[Abstract]:Objective to study the feasibility of using body surface electrode instead of multichannel esophageal electrode to measure respiratory center drive in patients with chronic obstructive pulmonary disease (COPD). Methods A total of 29 moderate and severe COPD patients (mean age: 65.6 鹵7.2) years were enrolled in this study. The changes of diaphragm electromyography and pulmonary ventilation volume during inhalation of bronchodiastolic agents and iso-carbon dioxide hyperventilation (MIV) were recorded by body surface electrode and multichannel electrode simultaneously. The difference of respiratory center drive between the two methods was compared. The variance analysis of repeated measurement data, the minimum LSD test for pairwise comparison, and Pearson test for correlation analysis were used. Results after inhaling bronchodiastolic agent, the pulmonary ventilation volume increased at rest and MIV, and the respiratory center drive measured by body surface electrode and multichannel esophagus electrode decreased synchronously, and there was a significant correlation between the lung ventilation volume and the respiratory center drive measured by body surface electrode and multichannel esophagus electrode. Conclusion when some measures are taken to minimize the interference of body surface electrodes, body surface electrodes can replace multichannel esophageal electrodes to determine the respiratory center drive in patients with chronic obstructive pulmonary disease.
