【摘要】：目的應用2002年歐洲腸外腸內(nèi)營養(yǎng)學會發(fā)布營養(yǎng)不良篩查指南(NRS-2002)調(diào)查住院慢性阻塞性肺疾病(COPD)患者營養(yǎng)不良發(fā)生情況,探討NRS-2002營養(yǎng)評價結果與體格檢查指標和生化指標的相關性。方法隨機抽取COPD患者218例,用NRS-2002進行營養(yǎng)風險篩查,同時進行體格檢查,計算對應指標,測定血生化指標,分析NRS-2002營養(yǎng)評價結果與體格檢查指標和生化指標的相關性。結果 218例住院COPD患者營養(yǎng)不良(NRS-2002≥3分)發(fā)生率31.7%,不同年齡段和不同肺功能分級患者間營養(yǎng)不良發(fā)生率存在統(tǒng)計學差異(P0.05)。NRS-2002營養(yǎng)評價結果與2個體格檢查指標[體質指數(shù)(BMI)和去脂體重指數(shù)(FFMI)]和2個生化指標[白蛋白(ALB)和前白蛋白(PA)]存在關聯(lián)性(P0.05)。結論住院COPD患者營養(yǎng)不良發(fā)生率較高,老年患者尤甚。NRS-2002適合作為住院COPD患者營養(yǎng)風險篩查工具,但在臨床應用中應結合體格檢查指標和生化指標進行全面綜合營養(yǎng)評估。
[Abstract]:Objective to investigate the prevalence of malnutrition in hospitalized patients with chronic obstructive pulmonary disease (COPD) by using the 2002 European Society of Parenteral Nutrition (EPN) guidelines for the screening of malnutrition (NRS-2002). To explore the correlation between the results of NRS-2002 nutrition evaluation and physical examination and biochemical indexes. Methods 218 patients with COPD were randomly selected for nutritional risk screening with NRS-2002. At the same time, the corresponding indexes were calculated and blood biochemical indexes were measured. Results the results of nutritional evaluation of NRS-2002 were correlated with physical examination and biochemical indexes. Results the incidence of malnutrition in 218 COPD patients was 31.7, and the incidence of malnutrition was 31.7 among patients with different age groups and different grade of pulmonary function. There was a significant difference in birth rate between P0.05N, NRS-2002 nutritional evaluation results and two physical examination indexes (body mass index (BMI), body mass index (BMI) and body mass index (BMI)), and two biochemical indexes (Alb and PAA). Conclusion there is a significant correlation between the nutritional evaluation results of P0.05and COPD patients in our hospital. The incidence of malnutrition is high, NRS-2002 is suitable for nutritional risk screening of hospitalized COPD patients, but comprehensive nutritional assessment should be carried out in clinical application combined with physical examination and biochemical indexes.
