【摘要】：目的研究2009-2013年天津市參保慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者住院費用內(nèi)部影響因素及發(fā)展趨勢,為合理控制醫(yī)療費用、完善醫(yī)療改革提供參考。方法采用新灰色關(guān)聯(lián)法分析住院各項費用關(guān)聯(lián)程度;運用結(jié)構(gòu)變動值、結(jié)構(gòu)變動度和結(jié)構(gòu)變動貢獻率等評價指標,分析5年間住院各項費用變化情況及其對總費用變化的影響程度。結(jié)果 5年間患者住院費用中,藥品費關(guān)聯(lián)度最大(1.0000),其次是檢查費(0.7232)、治療費(0.6841)和醫(yī)用材料費(0.6438);藥品費結(jié)構(gòu)變動貢獻率最大(36.78%),占住院總費用比例下降,其次是檢查費(32.48%)和醫(yī)用材料費(8.61%),占住院總費用比例上升,三者累計貢獻率達77.87%。結(jié)論控制藥品費應作為減少COPD患者住院費用負擔的關(guān)鍵突破口,加強高值醫(yī)用材料監(jiān)管,重視醫(yī)護人員勞動價值,進而優(yōu)化住院費用結(jié)構(gòu)。
[Abstract]:Objective to study the internal influencing factors and development trend of hospitalization expenses of patients with chronic obstructive pulmonary disease (pulmonary) in Tianjin from 2009 to 2013, in order to control the medical expenses reasonably. Methods the new grey correlation method was used to analyze the correlation degree of inpatient expenses, the structural change value, the structural change degree and the contribution rate of structural change were used. The changes of hospitalization expenses and their influence on total costs were analyzed. The most related degree of drug fee was 1.0000m, followed by examination fee (0.7232n), treatment fee (0.6841) and medical material fee (0.6438), the biggest contribution rate of structure change of drug fee was 36.78m, the proportion of total hospitalization cost decreased, the next was examination fee (32.488m) and medical material cost 8.611m, which accounted for the proportion of total hospitalization expenses. Conclusion the control of drug fees should be a key breakthrough to reduce the burden of hospitalization expenses of COPD patients, strengthen the supervision of high value medical materials, attach importance to the labor value of medical personnel, and then optimize the structure of hospitalization expenses.
