【摘要】:目的:通過分析結(jié)核性與惡性胸液?jiǎn)蝹(gè)核細(xì)胞(Pleural fluid mononuclear cells,PFMCs)在受到結(jié)核分枝桿菌特異性抗原肽ESAT-6(early secreted antigenic target-6)和CFP-10(culture filtrate protein-10)刺激后表達(dá)白細(xì)胞介素-22(Interleukin-22,IL-22)的特點(diǎn),探索結(jié)核性和惡性胸水的鑒別診斷新方法。 方法:采用e-Bioscience的IL-22和干擾素-γ(Interferon-γ,IFN-γ)試劑盒和流式細(xì)胞術(shù)微球陣列法,對(duì)52例結(jié)核性胸水和35例惡性胸水中經(jīng)結(jié)核分枝桿菌特異性抗原肽刺激后PFMCs所表達(dá)的IL-22和IFN-γ濃度進(jìn)行測(cè)定,并作統(tǒng)計(jì)學(xué)分析。 結(jié)果:經(jīng)結(jié)核桿菌特異抗原肽刺激,,結(jié)核性胸水上清中PFMC的IL-22表達(dá)水平顯著升高(P<0.05),且高于惡性胸水(P<0.05)。結(jié)核性胸膜炎的結(jié)核桿菌特異性IL-22在胸液上清PFMC中的表達(dá)水平與IFN-γ顯著相關(guān)(相關(guān)系數(shù)r=0.3485,P=0.0320)。ROC曲線顯示,結(jié)核桿菌特異性IL-22用于結(jié)核性胸膜炎的診斷敏感性、特異性和準(zhǔn)確性分別為90.38%、80.56%和85.23%,其診斷效能與IFN-γ相當(dāng)(P>0.05)。 結(jié)論:結(jié)核分枝桿菌抗原特異性IL-22可作為鑒別診斷胸腔積液的新指標(biāo)。
[Abstract]:Objective : To investigate the characteristics of interleukin - 22 ( IL - 22 ) after stimulation of Mycobacterium tuberculosis specific antigen peptide ( ESAT - 6 ) and P - 10 ( culture filtrate protein - 10 ) by analyzing tuberculous and malignant pleural fluid mononuclear cells ( PFs ) , and to explore new methods for differential diagnosis of tuberculous and malignant pleural effusion . Methods : The concentrations of IL - 22 and IFN - 緯 in 52 cases of tuberculous pleural effusion and 35 cases of malignant pleural effusion were measured with e - bioscience IL - 22 and interferon - 緯 ( interferon - 緯 , IFN - 緯 ) kit and flow cytometry . Results : The level of IL - 22 expression of PFMC was significantly increased ( P & lt ; 0.05 ) in the tuberculous pleural fluid by Mycobacterium tuberculosis specific antigen peptide stimulation ( P & lt ; 0.05 ) , and the expression level of TB - specific IL - 22 in pleural fluid supernatant was significantly correlated with IFN - 緯 ( r = 0.3485 , P = 0.0320 ) . ROC curves showed that the diagnostic sensitivity , specificity and accuracy of mycobacterium tuberculosis specific IL - 22 were 90.38 % , 80.56 % and 85.23 % , respectively , and their diagnostic efficacy was comparable with that of IFN - 緯 ( P > 0.05 ) . Conclusion : Mycobacterium tuberculosis antigen - specific IL - 22 can be used as a new index for differential diagnosis of pleural effusion .
