【摘要】：[目的]探討上海市不同孕期孕婦的雙酚A(BPA)暴露現(xiàn)狀及其與甲狀腺功能指標的關聯(lián)。[方法]收集上海市7個區(qū)共210名孕婦(孕早、中、晚期各三分之一)的尿樣,采用固相萃取-高效液相色譜串聯(lián)質(zhì)譜法(HPLC-MS/MS)測定尿液總BPA質(zhì)量濃度,并檢測血清中5個甲狀腺功能指標的水平:游離三碘甲狀腺原氨酸(FT3)、游離甲狀腺素(FT4)、促甲狀腺素(TSH)、甲狀腺球蛋白抗體(TGAb)、甲狀腺過氧化物酶抗體(TPOAb)。了解不同孕期孕婦的BPA暴露水平,采用Pearson相關及多元線性回歸分析尿BPA與血清甲狀腺功能指標的關聯(lián)。[結(jié)果]被檢孕婦的尿液BPA檢出率為95.7%,質(zhì)量濃度范圍為未檢出~11.99μg/L,整體水平呈偏態(tài)分布,經(jīng)尿比重校正后BPA質(zhì)量濃度M(P_(25),P_(75))為0.53(0.36,0.83)μg/L,孕早、中、晚期BPA質(zhì)量濃度差異無統(tǒng)計學意義。通過Pearson相關分析發(fā)現(xiàn),孕早、晚期經(jīng)尿比重校正的尿BPA質(zhì)量濃度與血清TGAb水平呈正相關(r=0.396,P=0.002;r=0.440,P=0.000)。調(diào)整年齡、孕中BMI、孕周這些協(xié)變量后,兩者仍呈正相關(b=0.369,95%CI:0.206~0.532,P=0.000)。尿BPA與其他甲狀腺功能指標無相關性。[結(jié)論]上海市孕婦普遍存在BPA暴露,孕期BPA暴露可能影響血清TGAb水平。
[Abstract]:[objective] to investigate the present status of bisphenol A (BPA) exposure and its association with thyroid function in pregnant women with different pregnancies in Shanghai. [methods] urine samples of 210 pregnant women (1/3 in the first trimester, 1/3 in the second trimester and 1/3 in the late trimester) were collected from 7 districts of Shanghai. The total BPA concentration in urine was determined by solid phase extraction (SPE) and high performance liquid chromatography tandem mass spectrometry (HPLC-MS/MS). The levels of free triiodothyronine (FT3), free thyroxine (FT4) and thyrotropin (TSH), thyroglobulin (TGAb),) in serum were determined. Thyroid peroxidase antibody (TPOAb). To investigate the level of BPA exposure in pregnant women during different pregnancy, the correlation between urinary BPA and serum thyroid function index was analyzed by Pearson correlation and multivariate linear regression analysis. [results] the detection rate of urine BPA in pregnant women was 95.70.The range of mass concentration was from 11.99 渭 g / L to 11.99 渭 g / L, and the whole level was skewed. The BPA concentration M (P25) was corrected by urine specific gravity. P _ (75) was 0.53 (0.36 鹵0.83) 渭 g / L, and there was no significant difference in BPA concentration in early, middle and late pregnancy. By Pearson correlation analysis, it was found that there was a positive correlation between urinary BPA concentration corrected by specific gravity in early and late pregnancy and serum TGAb level (r = 0.396p ~ (0.002) P ~ (0.002) P ~ (0.440) P ~ (0.000). After adjusting the age, the covariables of BMI, in pregnancy were positively correlated with each other (CI: 0.206 ~ 0.532). There was no correlation between urinary BPA and other thyroid function indexes. [conclusion] BPA exposure is common in pregnant women in Shanghai, and BPA exposure during pregnancy may affect serum TGAb level.
