【摘要】：目的探討妊娠期高血壓的危險(xiǎn)因素,為預(yù)防妊娠期高血壓發(fā)病提供參考依據(jù)。方法采用1∶2配對(duì)病例對(duì)照研究方法,入選2010年3月至2013年3月福建醫(yī)科大學(xué)附屬第一醫(yī)院住院分娩的妊娠期高血壓患者372例(病例組)和同期足月分娩健康新生兒的非妊娠期高血壓者744名(對(duì)照組)作為研究對(duì)象。采用問(wèn)卷調(diào)查和查閱臨床資料相結(jié)合的方式,收集兩組的基礎(chǔ)資料,并對(duì)危險(xiǎn)因素進(jìn)行單因素和多因素非條件Logistic回歸分析。結(jié)果與對(duì)照組相比,病例組年齡較大;24h尿白蛋白含量、血肌酐水平較高;孕前體質(zhì)量指數(shù)(BMI)≥24kg/m2、糖化血紅蛋白(HbA1c)在6.0%~6.5%、孕婦情緒焦慮和妊娠≥2胎者所占的比例較高;孕期檢查≥6次所占的比例較低(均P0.05)。單因素Logistic回歸分析顯示:年齡、孕前BMI、HbA1c、24h尿白蛋白含量、血肌酐水平、孕期檢查次數(shù)、孕婦情緒、多胎妊娠是妊娠期高血壓的危險(xiǎn)因素(均P0.05)。多因素非條件Logistic顯示:年齡(OR=1.317)、孕前BMI(OR=1.183)、HbA1c(OR=1.303)、24h尿白蛋白(OR=2.847)、血肌酐(OR=1.102)是妊娠期高血壓發(fā)生的獨(dú)立危險(xiǎn)因素。結(jié)論年齡較大、孕前BMI≥24kg/m2、HbA1c在6.0%~6.5%、24h尿白蛋白含量和血肌酐水平較高的孕婦,容易發(fā)生妊娠期高血壓。
[Abstract]:Objective to explore the risk factors of hypertension complicating pregnancy and to provide reference for the prevention of hypertension complicating pregnancy. Methods the 1:2 matched case-control study was used.
