【摘要】：目的探討不同分娩方式對(duì)足月初產(chǎn)婦產(chǎn)后陰道收縮力、盆底功能及性生活質(zhì)量的影響。方法選擇足月初產(chǎn)婦733例為研究對(duì)象,根據(jù)產(chǎn)婦分娩方式分為會(huì)陰側(cè)切組(154例)、順產(chǎn)組(430例)、剖宮產(chǎn)組(149例)。產(chǎn)后6周、3個(gè)月分別測(cè)量陰道收縮力和盆底肌力,并咨詢性生活恢復(fù)時(shí)間及有無(wú)疼痛感及尿失禁等。結(jié)果產(chǎn)后6周和產(chǎn)后3個(gè)月陰道收縮力測(cè)量,會(huì)陰側(cè)切組與順產(chǎn)組之間比較差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05),會(huì)陰側(cè)切組、順產(chǎn)組與剖宮產(chǎn)組之間比較差異有統(tǒng)計(jì)學(xué)意義(P0.05),剖宮產(chǎn)組陰道收縮力高于會(huì)陰側(cè)切組和剖宮產(chǎn)組。3組產(chǎn)后3個(gè)月與6周比較差異均有統(tǒng)計(jì)學(xué)意義(P0.01)。剖宮產(chǎn)組恢復(fù)性生活的時(shí)間明顯高于會(huì)陰側(cè)切組和順產(chǎn)組(P0.05)。順產(chǎn)組產(chǎn)后1周會(huì)陰水腫發(fā)生率明顯低于會(huì)陰側(cè)切組(P0.05),剖宮產(chǎn)組無(wú)會(huì)陰水腫發(fā)生。3組首次性生活疼痛發(fā)生率比較差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。剖宮產(chǎn)組術(shù)后6周盆底肌力得分明顯高于會(huì)陰側(cè)切組和順產(chǎn)組(P0.05)。結(jié)論應(yīng)限制會(huì)陰側(cè)切的應(yīng)用,嚴(yán)格掌握剖宮產(chǎn)指征,在孕婦自愿的前提下可選擇剖宮產(chǎn)。
[Abstract]:Objective to investigate the effects of different delivery modes on postpartum vaginal contractility, pelvic floor function and sexual quality. Methods 733 term primipara were divided into perineal resection group (154 cases), normal delivery group (430 cases) and cesarean section group (149 cases). The vaginal contractility and pelvic floor muscle strength were measured at 6 weeks and 3 months postpartum, and the recovery time of sexual life and the feeling of pain and urinary incontinence were consulted. Results there was no significant difference in vaginal contractility between the perineal lateral resection group and the normal delivery group at 6 weeks postpartum and 3 months postpartum (P0.05), but there was no significant difference between the perineal side resection group and the normal delivery group (P0.05). The vaginal contractility of caesarean section group was higher than that of perineal lateral incision group and cesarean section group (P0.05). There were significant differences in postpartum 3 months and 6 weeks postpartum in caesarean section group (P0.01). The recovery time of sexual life in cesarean section group was significantly higher than that in perineal resection group and normal delivery group (P0.05). The incidence of perineal edema in the normal delivery group was significantly lower than that in the perineal lateral resection group (P0.05), while there was no significant difference in the incidence of first sexual pain in the cesarean section group without perineal edema (P0.05). The pelvic floor muscle strength score in cesarean section group was significantly higher than that in perineal lateral incision group and spontaneous delivery group (P0.05). Conclusion the application of lateral perineal resection should be restricted and the indications of cesarean section should be strictly grasped.
