【摘要】：目的:探討宮頸門診就診人群中人乳頭瘤病毒型別分布及其與年齡、季節(jié)相關(guān)性并分析病毒清除所需時(shí)間,為宮頸疾病防治提供理論依據(jù)。方法:回顧性分析南京鼓樓醫(yī)院婦科宮頸門診2318例婦女,自愿行HPV分型結(jié)果,探討HPV感染與年齡及季節(jié)的相關(guān)性,并挑選其中首次發(fā)現(xiàn)HPV陽(yáng)性,3月后定期復(fù)查HPV分型直至分型轉(zhuǎn)陰,分析其HPV清除所需時(shí)間。結(jié)果:1、2318例行HPV分型檢測(cè)結(jié)果:陰性908例,陽(yáng)性1410例(60.83%),高危HPV感染型別主要為:HPV16、58、52、31、53及HPV33,低危HPV感染型別主要為 HPV81(CP8304)、11及HPV6;2、高危HPV在各年齡組中陽(yáng)性率不同,1)≤30歲組共537例,高危HPV陽(yáng)性 285 例(53.07%);2)31-40 歲組共 791 例,高危 HPV 陽(yáng)性 431 例(54.49%);3)41-50歲組共642例,高危HPV陽(yáng)性346例(53.89%);4)51-60歲組共272例,高危HPV陽(yáng)性121例(44.49%);5)≥61歲組共76例,高危HPV陽(yáng)性28例(36.84%)。經(jīng)各組間高危HPV陽(yáng)性率比較發(fā)現(xiàn),≤30歲組、31-40歲組、41-50歲組,分別與51-60歲組、≥61歲組差異具有統(tǒng)計(jì)學(xué)意義,其余差異無統(tǒng)計(jì)學(xué)意義;3、在不同季節(jié)中高危HPV檢出率不同:春季共766例,高危HPV檢出陽(yáng)性442例(57.70%);夏季共628例,高危HPV檢出陽(yáng)性356例(56.69%);秋季共413例,高危HPV檢出陽(yáng)性228例(55.21%);冬季共511例,高危HPV檢出陽(yáng)性254例(49.71%),各季節(jié)間高危HPV檢出率比較發(fā)現(xiàn),冬季高危HPV檢出率分別與春季、夏季間高危HPV檢出率比較,P0.05,差異有統(tǒng)計(jì)學(xué)意義,其余季節(jié)間差異無統(tǒng)計(jì)學(xué)意義;4、四個(gè)季節(jié)中HPV型別檢出情況:1)春季主要檢出HPV型別:HPV16(12.92%、HPV58(12.14%)、HPV52(9.92%),HPV31(7.31%);2)夏季主要檢出 HPV型別:HPV16(16.88%),HPV58(9.55%),HPV52(9.39%),HPV18(6.69%);3)秋季主要檢出HPV型別:HPV16(16.22%),HPV58、HPV52(各10.65%),HPV33(6.30%),HPV18(4.73%);4)冬季主要檢出 HPV 型別:HPV16(10.57%),HPV58(10.18%),HPV52(8.81%),HPV31、HPV53(各5.68%);5、在研究對(duì)象中挑選首次發(fā)現(xiàn)HPV陽(yáng)性,3月后定期復(fù)查HPV分型直至HPV分型轉(zhuǎn)陰的婦女共99例,72例高危HPV感染其平均清除時(shí)間268.53天,其中HPV16平均清除時(shí)間272.23天。27例低危HPV感染其平均清除時(shí)間219.19天。99例HPV感染婦女中超過50%其病毒清除時(shí)間為3-12個(gè)月,中位清除時(shí)間為182 天。結(jié)論:1、南京鼓樓醫(yī)院宮頸門診就診人群中高危HPV感染主要型別為:HPV16、58、52、31、53 及 HPV33;低危 HPV 型別為:HPV81(CP8304)、11 及 HPV6;2、高危HPV感染在各年齡組中有所不同,≤50歲高危HPV感染比例較高,51歲以后略有下降;3、高危HPV在各季節(jié)中陽(yáng)性檢出率存在差異:春季、夏季高危HPV檢出率分別高于冬季,其余季節(jié)間無差異;4、各季節(jié)中高危HPV感染型別略有差異,HPV16/58/52是所有季節(jié)都常見的高危型別,除以上型別外,HPV31在春季較常見,HPV18在夏季常見,HPV33在秋季常見;5、本研究中HPV陽(yáng)性并定期復(fù)查分型婦女中超過50%其病毒清除時(shí)間為3-12個(gè)月,中位清除時(shí)間為182天。
[Abstract]:Objective: to study the distribution of human papillomavirus (HPV) type and its correlation with age, season and time of virus clearance in cervical outpatient population, and to provide theoretical basis for the prevention and treatment of cervical diseases. Methods: the results of HPV typing in 2318 women from gynecologic cervical outpatient department of Nanjing Gulou Hospital were analyzed retrospectively, and the correlation between HPV infection and age and season was discussed. The HPV positive was found for the first time, and the time of HPV clearance was analyzed by reviewing HPV typing regularly until the type turned negative 3 months later. Results the results of HPV typing were as follows: 908 cases were negative, 1410 cases were positive (60.83%).
