【摘要】：目的:比較超聲和磁共振成像(MRI)對(duì)兇險(xiǎn)型前置胎盤(pán)(Pernicious Placenta Previa,PPP)伴胎盤(pán)植入的診斷效能。方法:選擇2011年1月至2014年12月在永川婦產(chǎn)醫(yī)院住院的33例可疑胎盤(pán)植入PPP患者為研究對(duì)象。所有患者均行超聲和MRI檢查,并于剖宮產(chǎn)終止妊娠后行病理檢查。以手術(shù)-病理檢查結(jié)果為金標(biāo)準(zhǔn),比較超聲與MRI檢查對(duì)PPP伴植入的診斷效能。結(jié)果:經(jīng)手術(shù)-病理檢查,26例PPP伴植入(包括:粘連性8例,植入性15例,穿透性3例),7例不伴植入。MRI檢查診斷PPP伴植入的準(zhǔn)確率(87.88%)略高于超聲檢查(84.85%),但兩種方法間比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:超聲檢查診斷PPP伴植入的準(zhǔn)確率與MRI接近,但重復(fù)性更好,價(jià)格更便宜,可作為本病診斷的首選方法,而將MRI作為輔助檢查手段。
[Abstract]:Objective: to compare the diagnostic efficacy of ultrasound and magnetic resonance imaging (MRI) in the diagnosis of (Pernicious Placenta Previa previa PPP with placenta accreta. Methods: 33 patients with suspected placenta accreta (PPP) who were hospitalized in Yongchuan Obstetrics and Gynecology Hospital from January 2011 to December 2014 were selected as study subjects. Ultrasound and MRI were performed in all patients, and pathological examination was performed after caesarean section. The diagnostic efficacy of ultrasound and MRI in the diagnosis of PPP with implantation was compared with the results of surgical-pathological examination as gold standard. Results: 26 cases of PPP with implantation (including 8 cases of adhesiveness, 15 cases of implantation) were examined by surgical and pathological examination. The diagnostic accuracy of PPP with implantation was 87.88% in 7 cases (87.88%) higher than that in ultrasound (84.85%), but there was no significant difference between the two methods (P0.05). Conclusion: the accuracy of ultrasound diagnosis of PPP with implantation is close to that of MRI, but the reproducibility is better and the price is lower, so it can be used as the first choice for the diagnosis of this disease, and MRI is used as the auxiliary examination method.
