【摘要】：目的探討妊娠及產(chǎn)褥期婦女并發(fā)下肢深靜脈血栓溶栓治療的臨床療效。方法選擇2012年3月至2015年7月在河南省鄭州市第十五人民醫(yī)院婦產(chǎn)科住院治療的69例妊娠及產(chǎn)褥期并發(fā)下肢深靜脈血栓患者作為研究對(duì)象,均給予尿激酶靜脈溶栓治療以及肝素抗凝治療,比較其治療前及治療后3、7 d的各項(xiàng)凝血功能指標(biāo)、患肢腫脹程度和各項(xiàng)下肢阻抗容積波圖指標(biāo)變化情況。結(jié)果治療前及治療后3、7 d患者的Fi B呈顯著下降趨勢(shì),而PT、APTT均呈顯著升高趨勢(shì)(均P0.001);治療后3、7 d患者的股骨中段周徑、脛骨中段周徑均顯著減小(均P0.001);與健側(cè)相比,患者患肢腫脹程度明顯改善(P0.05);治療前及治療后3、7 d患者的IPG升高值、IPG下降值、IPG下降值/IPG升高值、阻抗變化比率均呈顯著升高趨勢(shì)(均P0.001)。結(jié)論溶栓治療快速溶解下肢血栓、改善其靜脈回流,對(duì)于妊娠及產(chǎn)褥期并發(fā)下肢深靜脈血栓患者具有較為明顯的臨床療效。
[Abstract]:Objective to investigate the clinical effect of thrombolytic therapy for lower extremity deep vein thrombosis in pregnant and puerperium women. Methods 69 cases of pregnancy and puerperium complicated with deep venous thrombosis of lower extremity were selected from March 2012 to July 2015 in the 15th people's Hospital of Zhengzhou, Henan Province. All patients were treated with urokinase intravenous thrombolytic therapy and heparin anticoagulant therapy. The indexes of coagulation function, swelling degree of affected limb and impedance volume wave of lower extremity were compared before and after 3 to 7 days of treatment. Results the FI-B of patients before and after treatment decreased significantly, but PTT APTT increased significantly (all P0.001), and the diameter of femur and middle tibia decreased significantly (P0.001), compared with healthy side, the diameter of midfemur and middle tibia decreased significantly (P0.001). The swelling degree of the affected limbs was significantly improved (P0.05), and the IPG decreased value and the ratio of impedance change were significantly increased before treatment and 3 days after treatment (all P0.001). Conclusion Thrombolytic therapy for rapid dissolution of lower extremity thrombus and improvement of venous reflux is effective in pregnancy and puerperium complicated with deep venous thrombosis of lower extremity.
