【摘要】：目的探討多普勒超聲對妊娠期高血壓患者子宮動脈血流的檢測情況。方法選擇我院2015年2月至2016年2月我院接診的80例妊娠期高血壓患者作為觀察組,并選擇同期我院接診的80例正常妊娠產婦作為對照組。使用美國GE Voluson E8超聲多普勒血流檢測儀,比較兩組子宮動脈血流及妊娠結局;并根據(jù)是否出現(xiàn)舒張早期切跡將觀察組分為切跡組和無切跡組,比較兩組妊娠結局。結果觀察組阻力指數(shù)、搏動指數(shù)、收縮-舒張流速比值均比對照組高[(0.59±0.06)vs(0.51±0.03),(0.95±0.18)vs(0.72±0.12),(2.47±0.52)vs(2.01±0.31)],差異均具有統(tǒng)計學意義(P0.05);觀察組新生兒體重明顯低于對照組,早產、剖宮產、胎兒窘迫明顯高于對照組,差異均具有統(tǒng)計學意義(P0.05);在觀察組中,舒張早期切跡發(fā)生率為45.00%(36/80),切跡組新生兒體重明顯低于無切跡組,早產、羊水異常、胎兒窘迫發(fā)生率均明顯高于無切跡組,差異均具有統(tǒng)計學意義(P0.05)。結論在妊娠期高血壓患者中應用多普勒超聲檢測子宮動脈血流動力學有助于預測不良妊娠結局,臨床應用價值高。
[Abstract]:Objective to investigate the detection of uterine artery flow in hypertensive women with gestational hypertension by Doppler ultrasound. Methods from February 2015 to February 2016, 80 pregnant women with hypertension complicating pregnancy were selected as observation group and 80 normal pregnant women in the same period as control group. The uterine artery blood flow and pregnancy outcome were compared by US GE Voluson E8 ultrasound Doppler flow detector, and the observed group was divided into notch group and no notch group according to whether there was early diastolic notch, and compared the pregnancy outcome between the two groups. Results the resistance index, pulsatility index and systolic / diastolic velocity ratio in the observation group were significantly higher than those in the control group [0.59 鹵0.06)vs(0.51 鹵0.03 鹵0.95 鹵0.12 鹵2.47 鹵0.31], the difference was statistically significant (P 0.05), the weight of the newborns in the observation group was significantly lower than that in the control group, preterm delivery, cesarean section, fetal distress were significantly higher than those in the control group. In the observation group, the incidence of early diastolic notch was 45.00% (36 / 80), the weight of newborns in the notch group was significantly lower than that in the no notch group, the incidence of premature delivery, amniotic fluid abnormality and fetal distress was significantly higher in the observation group than in the no notch group. The difference was statistically significant (P 0.05). Conclusion the application of Doppler ultrasound in the detection of uterine artery hemodynamics in hypertensive complicating pregnancy is helpful to predict the adverse pregnancy outcome, and the clinical application value is high.
