【摘要】：宮頸癌是女性第二常見的惡性腫瘤,發(fā)病率僅次于乳腺癌,且存在年青化趨向。淋巴結(jié)轉(zhuǎn)移狀況是評估宮頸癌預(yù)后的獨立危險因素,也是指導(dǎo)術(shù)后輔助治療的重要參考依據(jù)。前哨淋巴結(jié)檢測作為一種淋巴結(jié)轉(zhuǎn)移的篩選手段,廣泛用于乳腺癌診療中,近年被試用于宮頸癌,原因是廣泛性子宮切除+盆(腹)腔淋巴結(jié)清除(RHPLND)是早期宮頸浸潤癌的常用術(shù)式,而數(shù)據(jù)卻顯示早期宮頸癌的淋巴轉(zhuǎn)移率并不高,大部分早期宮頸癌患者被實施了不必要的系統(tǒng)性淋巴結(jié)清除,且有部分患者承受其帶來的相關(guān)并發(fā)癥。前哨淋巴結(jié)微轉(zhuǎn)移指前哨淋巴結(jié)內(nèi)常規(guī)病理學(xué)不能檢出的微小轉(zhuǎn)移灶及散播腫瘤細胞,目前在宮頸癌診療中的意義尚未完全闡明,其檢測作為淋巴結(jié)轉(zhuǎn)移狀態(tài)的一種診斷技術(shù),可提高宮頸癌前哨淋巴結(jié)檢測的準確性,旨在準確評估早期宮頸癌患者的淋巴結(jié)轉(zhuǎn)移狀態(tài),精準指導(dǎo)早期宮頸癌的個體化治療,但目前其檢測方法和意義均未得到大量數(shù)據(jù)證實,本文根據(jù)檢索各數(shù)據(jù)庫的相關(guān)文獻,就早期宮頸癌前哨淋巴結(jié)微轉(zhuǎn)移檢測方法以及臨床意義研究的最新進展加以綜述。
[Abstract]:Cervical cancer is the second most common malignant tumor in women. Lymph node metastasis is an independent risk factor for the prognosis of cervical cancer and an important reference for guiding postoperative adjuvant therapy. As a screening method for lymph node metastasis, sentinel lymph node detection is widely used in the diagnosis and treatment of breast cancer. The reason is that extensive hysterectomy of pelvic (abdominal) cavity lymphadenectomy is a common procedure for early invasive carcinoma of the cervix, but the data show that the lymphatic metastasis rate of early cervical cancer is not high. Most patients with early cervical cancer undergo unnecessary systemic lymphadenectomy, and some of them suffer associated complications. Sentinel lymph node micrometastasis refers to the small metastatic foci and tumor spreading cells which cannot be detected by routine pathology in sentinel lymph nodes. The significance of micrometastasis in diagnosis and treatment of cervical cancer has not been fully clarified at present. The detection of micrometastasis in sentinel lymph nodes is regarded as a diagnostic technique for lymph node metastasis. It can improve the accuracy of sentinel lymph node detection in cervical cancer. It aims to accurately evaluate the status of lymph node metastasis in patients with early cervical cancer and guide the individualized treatment of early cervical cancer. However, the methods and significance of detection have not been confirmed by a large number of data. In this paper, the methods of detection of sentinel lymph node micrometastasis in early cervical cancer and the latest research progress of clinical significance are reviewed according to the relevant literatures in various databases.
