【摘要】：目的探討子宮不典型息肉狀腺肌瘤(APA)的臨床特征。方法對38例APA的臨床資料進(jìn)行回顧性分析。結(jié)果 (1)B超檢查結(jié)果示:有月經(jīng)失調(diào)癥狀或不孕癥患者中,高達(dá)60.87%(14/23)的患者B超檢查無異常表現(xiàn);而在無癥狀的15例患者中,93.33%(14/15)的患者B超檢查提示為宮腔占位或子宮內(nèi)膜欠均。(2)共14例患者進(jìn)行了宮腔鏡檢查,其中單發(fā)息肉樣贅生物8例,多發(fā)息肉樣贅生物5例。(3)病理檢查提示44.74%(17/38)的患者周圍子宮內(nèi)膜表現(xiàn)為增生過長。(4)初治為經(jīng)宮腔鏡治療的10例患者,隨訪無一例復(fù)發(fā);初治為診斷性刮宮治療的27例患者,其復(fù)發(fā)率為22.22%(6/27)。癌變率5.26%(2/38)。有生育要求的3例患者已受孕分娩。結(jié)論APA好發(fā)于孕齡婦女,其發(fā)病可能與雌激素持續(xù)作用有關(guān),臨床表現(xiàn)及B超檢查均無特異性,APA有低度惡性潛能,治療后仍密切隨訪。
[Abstract]:Objective to investigate the clinical features of atypical polypoid adenomyoma of the uterus (APA). Methods the clinical data of 38 cases of APA were retrospectively analyzed. Results (1) the results of B ultrasound examination showed that there were no abnormal manifestations of B ultrasound in patients with menstrual disorders or infertility, up to 60.87% (14/23), and 93.33% (14/15) in asymptomatic 15 patients. (2) a total of 14 patients underwent hysteroscopy, including 8 cases of polyposis and 5 polyposis. (3) pathological examination suggested that the endometrium in the peripheral endometrium of 44.74% (17/38) patients were hyperproliferative. (4) 10 patients who had been treated by hysteroscopy (4) were followed up. One case was relapsed; the recurrence rate of 27 patients with diagnostic curettage was 22.22% (6/27). The rate of canceration was 5.26% (2/38). It was found that 3 patients with reproductive requirements had been pregnant and delivered. Conclusion APA is well developed in pregnant women, the incidence of which may be related to the continuous action of estrogen, the clinical manifestation and B ultrasonic examination are not specific, APA has low malignant potential, treatment and treatment. After the treatment, the follow-up was still closely followed.
