【摘要】：目的:分析青春期盆腔子宮內(nèi)膜異位癥(EMs)患者的臨床資料,探討早期發(fā)現(xiàn)、早期治療EMs的重要性。方法:收集我院1999年3月至2015年8月經(jīng)手術確診的195例青春期盆腔EMs患者的臨床資料,按r-AFS不同分期(I、II期和III、IV期)、有無梗阻性生殖道畸形、畸形組內(nèi)異癥嚴重程度分組,回顧分析各組的平均年齡、初潮年齡、痛經(jīng)、BMI、延遲診斷時間、彩超診斷準確性及誤診情況等。結(jié)果:III、IV期組患者的平均年齡、延遲診斷時間大于I、II期組,痛經(jīng)發(fā)生率及彩超診斷準確率高于I、II期組,合并生殖道畸形概率低于I、II期組,差異有統(tǒng)計學意義(P0.05)。生殖道畸形組患者的平均年齡、延遲診斷時間小于無畸形組,痛經(jīng)發(fā)生率、彩超診斷準確率、重度內(nèi)異癥比例低于無畸形組,誤診率高于無畸形組,差異有統(tǒng)計學意義(P0.05)。結(jié)論:盆腔EMs是青春期女性的一種嚴重進展性疾病,早期發(fā)現(xiàn)、診斷及治療是延緩疾病進展、保護青春期女性生育能力的重要途徑。
[Abstract]:Objective: to analyze the clinical data of pubertal pelvic endometriosis and explore the importance of early detection and early treatment of EMs. Methods: the clinical data of 195 patients with puberty pelvic EMs diagnosed by operation from March 1999 to August 2015 in our hospital were collected. According to the different stages of r-AFS, there were no obstructive genital tract malformations and severe degree of endometriosis in different stages of r-AFS. The average age, menarche age, dysmenorrhea BMIs, delayed diagnosis time, diagnostic accuracy and misdiagnosis were analyzed retrospectively. Results the average age, delayed diagnosis time, dysmenorrhea rate and accuracy rate of color Doppler ultrasonography were higher in the stage IV group than in the stage II group, and the probability of complicated genital tract malformation was lower than that in the stage II group, the difference was statistically significant (P 0.05). The average age, delayed diagnosis time, incidence of dysmenorrhea, diagnostic accuracy of color Doppler ultrasound, proportion of severe endoses were lower in the group of genital tract malformation than those in the group without malformation, the misdiagnosis rate was higher than that in the group without malformation, and the difference was statistically significant (P 0.05). Conclusion: pelvic EMs is a serious progressive disease in adolescent women. Early detection, diagnosis and treatment is an important way to delay the progression of disease and protect the reproductive ability of adolescent women.
