【摘要】：目的探討分娩前后凝血與纖溶指標的變化及及與炎癥因子的相關性。方法收集2014年1月至2016年6月在江蘇省無錫市婦幼保健院接受分娩產婦的臨床資料,根據分娩后出血情況分為產后出血組和正常分娩組。比較兩組產婦凝血、纖溶指標和炎癥細胞因子并分析參數間的相關性。結果分娩前,與正常分娩組比,產后出血組FIB明顯降低(P0.05),APTT、PT、TT和D-D明顯增高(P0.05),兩組IL-6、hs-CRP和TNF-α差異無統計學意義(P0.05);分娩后,與正常分娩組比,產后出血組FIB明顯降低(P0.05),APTT、PT、TT、D-D、IL-6、hs-CRP和TNF-α明顯增高(P0.05)。分娩前IL-6、hs-CRP和TNF-α與凝血、纖溶指標無相關性(P0.05);分娩后IL-6、hs-CRP和TNF-α與FIB呈負相關(P0.05),與APTT、PT、TT和D-D呈正相關(P0.05)。結論分娩前后,產后出血產婦的凝血和纖溶指標均有明顯變化,均與炎癥細胞因子存在相關性,可作為臨床監(jiān)測和預防產后出血的重要指標。
[Abstract]:Objective to investigate the changes of coagulation and fibrinolysis indexes before and after delivery and their correlation with inflammatory factors. Methods from January 2014 to June 2016, we collected the clinical data of parturient in Wuxi Maternal and Child Health Hospital, Jiangsu Province, and divided them into postpartum hemorrhage group and normal delivery group according to postpartum hemorrhage. The coagulation, fibrinolysis and inflammatory cytokines were compared between the two groups. Results before delivery, compared with the normal delivery group, the FIB in the postpartum hemorrhage group was significantly lower than that in the normal delivery group. The levels of FIB and D-D were significantly increased in the postpartum hemorrhage group. The levels of IL-6 hs-CRP and TNF- 偽 in the two groups were not significantly different from those in the normal delivery group (P 0.05). After delivery, FIB in the postpartum hemorrhage group was significantly lower than that in the normal delivery group, and the levels of FIB and TNF- 偽 were significantly higher in the postpartum hemorrhage group than in the normal delivery group. Before delivery, IL-6 hs-CRP and TNF- 偽 had no correlation with coagulation and fibrinolysis, but there was a negative correlation between IL-6 hs-CRP and TNF- 偽 and FIB (P0.05), and a positive correlation between IL-6 hs-CRP and D-D and PTTT and D-D. Conclusion the coagulation and fibrinolysis indexes of postpartum hemorrhage parturient have obvious changes before and after delivery, which are related to inflammatory cytokines, and can be used as an important index for clinical monitoring and prevention of postpartum hemorrhage.
