【摘要】：目的:探討常規(guī)磁共振成像(MRI)平掃結(jié)合彌散加權(quán)成像(DWI)在宮頸癌分期和病理特征等方面的價(jià)值。方法:選取宮頸癌患者56例,行MRI平掃及DWI序列掃描并且測(cè)量表觀擴(kuò)散系數(shù)(ADC)。將臨床婦科檢查、MRI、MRI聯(lián)合DWI分期與術(shù)后病理分期比較,分析宮頸癌病理類型、宮頸鱗癌分化程度、宮頸鱗癌分期與ADC值的關(guān)系。結(jié)果:對(duì)Ⅰ_b期、Ⅱ_a期的診斷,MRI平掃結(jié)合DWI最具優(yōu)勢(shì),宮頸鱗癌的平均ADC值小于腺癌,鱗癌隨著分化程度的增高平均ADC值增加,以上結(jié)果差異均具有統(tǒng)計(jì)學(xué)意義(P0.05),對(duì)宮頸鱗癌Ⅰ_a、Ⅰ_b、Ⅱ_a期三組平均ADC值進(jìn)行方差分析,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:MRI聯(lián)合DWI對(duì)宮頸癌的分期最有優(yōu)勢(shì),ADC值對(duì)宮頸癌病理類型及鱗癌的分化程度具有指導(dǎo)作用,但是對(duì)鱗癌的病理分期無(wú)價(jià)值。
[Abstract]:Objective: To investigate the conventional magnetic resonance imaging (MRI) scan combined with diffusion weighted imaging (DWI) in cervical cancer staging and pathological feature value. Methods: 56 cases of patients with cervical cancer, underwent MRI scan and DWI sequence scanning and apparent diffusion coefficient measurement table (ADC). The clinical gynecological examination, MRI comparison, MRI staging and postoperative pathologic findings combined with DWI analysis of cervical cancer staging, pathological type, degree of differentiation of cervical squamous cell carcinoma, the relationship between cervical squamous cell carcinoma staging and ADC. Results: on stage I _b, the diagnosis of stage II _a, MRI scan combined with DWI the most advantage, the average value is less than ADC in cervical squamous cell carcinoma. With the increase of the average degree of differentiation of ADC squamous cell carcinoma of the added value of the above results, the differences were statistically significant (P0.05), the cervical squamous cell carcinoma I _a, I _b, phase II _a three group of average ADC value variance analysis, the difference was not statistically significant (P0.05). Conclusion: MRI combined with DWI on the stage of cervical cancer Advantage, ADC value can guide the pathological type of cervical cancer and the degree of differentiation of squamous cell carcinoma, but it has no value to the pathological stage of squamous cell carcinoma.
