【摘要】：目的評(píng)價(jià)國(guó)內(nèi)6個(gè)標(biāo)準(zhǔn)的雙頂徑與孕齡對(duì)照表對(duì)唐氏綜合征篩查整體效果的影響和對(duì)孕婦個(gè)體風(fēng)險(xiǎn)率變化的影響。方法采用6種雙頂徑與孕齡對(duì)照表估計(jì)25 346例孕12~20周孕婦的孕齡,孕齡計(jì)算精確到天,計(jì)算6種對(duì)照表的產(chǎn)前篩查風(fēng)險(xiǎn)值,通過受試者工作特征曲線(ROC)曲線下面積、檢出率、假陽(yáng)性率來評(píng)價(jià)不同對(duì)照表對(duì)篩查的影響;通過個(gè)體風(fēng)險(xiǎn)率的變化來評(píng)價(jià)孕齡偏差對(duì)孕婦個(gè)體的影響。結(jié)果 ROC曲線下面積最高為自建對(duì)照表(0.972),最低為上海某醫(yī)院對(duì)照表(0.923),二者間差異有統(tǒng)計(jì)學(xué)意義(P=0.045)。在同樣75%檢出率條件下,假陽(yáng)性率最低為自建對(duì)照表(2.3%),截?cái)嘀禐?∶280;最高為《中華婦產(chǎn)科學(xué)》對(duì)照表(4.8%),截?cái)嘀禐?∶345。對(duì)于孕婦個(gè)體風(fēng)險(xiǎn)值,估計(jì)的孕齡比真實(shí)值偏大,風(fēng)險(xiǎn)將人為的變大,假陽(yáng)性的風(fēng)險(xiǎn)增大;估計(jì)的孕齡比真實(shí)值偏小,風(fēng)險(xiǎn)將人為的變小,假陰性的風(fēng)險(xiǎn)增大。結(jié)論 6種對(duì)照表對(duì)唐氏綜合征的篩查有不同的影響,自建對(duì)照表估計(jì)孕齡篩查效果最好,香港對(duì)照表估計(jì)孕齡篩查效果第二,不真實(shí)的孕齡估計(jì)對(duì)孕婦個(gè)體風(fēng)險(xiǎn)值有較大的影響。
[Abstract]:Objective to evaluate the effect of six standard diameters of parietal diameter and gestational age on the overall screening effect of Down's syndrome and the change of individual risk rate of pregnant women.Methods the gestational age of 25 346 pregnant women at 1220 weeks was estimated by using six kinds of biparietal diameters and gestational age comparison tables. The pregnancy age was calculated accurately to the day, and the prenatal screening risk values were calculated. The area under the ROC curve was calculated by the operating characteristic curve of the subjects and the detectable rate was detected.The false positive rate was used to evaluate the influence of different comparison tables on screening, and the influence of pregnancy age deviation on pregnant women was evaluated by the change of individual risk rate.Results the area under the ROC curve was the highest in the self-made control table 0.972 and the lowest in a hospital in Shanghai 0.923. The difference between the two was statistically significant.Under the condition of the same 75% positive rate, the lowest false positive rate was the self-made control table 2.30.The truncation value was 1: 280, and the highest was 4.8and the truncation value was 1: 345in the "Chinese obstetrics and gynecology" control table.For the individual risk value of pregnant women, the estimated gestational age is larger than the true value, the risk will increase artificially, the risk of false positive will increase, the estimated gestational age will be smaller than the true value, the risk will be artificially small, the risk of false negative will increase.Conclusion there are different effects on the screening of Down's syndrome by six kinds of comparison tables. The self-made comparison table has the best effect in estimating the gestational age, and the Hong Kong comparison table is the second in estimating the screening effect of gestational age.The unreal estimation of gestational age has great influence on the individual risk value of pregnant women.
