【摘要】：目的:探討附件扭轉臨床表現、體征及相關實驗室指標與扭轉附件伴壞死的相關性及其預測價值。方法:回顧性分析2013年1月至2016年5月98例于重慶醫(yī)科大學附屬第一醫(yī)院經手術治療的附件扭轉患者的臨床資料,以術后病檢結果為金標準,將其分為壞死組(45例),非壞死組(53例),對兩組患者的臨床資料進行統(tǒng)計學分析。結果:壞死組體溫、FIB及附件扭轉度數均高于非壞死組(P0.05)。FIB、體溫及扭轉度數與扭轉附件壞死的spearman相關系數分別為0.246,P=0.014;0.290,P=0.004及0.306,P=0.002。體溫、FIB及扭轉度數對扭轉附件壞死預測的ROC曲線的AUC分別為0.668,P=0.004;0.643,P=0.015和0.670,P=0.004。三者聯合對于扭轉附件壞死預測的AUC為0.754,P=0.000。結論:壞死組體溫、FIB及附件扭轉度數均高于非壞死組。體溫、FIB及扭轉度數對于扭轉附件壞死均具有一定預測性,但預測價值相對較低;三個指標聯合預測價值優(yōu)于單指標預測價值。
[Abstract]:Objective: to investigate the correlation and predictive value of clinical manifestations, signs and laboratory indexes of accessory torsion with necrosis.Methods: the clinical data of 98 patients with accessory torsion treated in the first affiliated Hospital of Chongqing Medical University from January 2013 to May 2016 were retrospectively analyzed.It was divided into necrotic group (n = 45) and non-necrotic group (n = 53).The AUC of the ROC curves predicted by body temperature and torsion degree for torsion adnexal necrosis were 0. 668 P0. 004 and 0. 643 P0. 015 and 0. 670 P0. 004, respectively.The combined AUC for torsion adnexal necrosis was 0. 754P0. 000.Conclusion: the FIB and the degree of accessory torsion in necrotic group were higher than those in non-necrotic group.Both the FIB and the degree of torsion had a certain predictive value for torsional accessory necrosis, but the predictive value was relatively low, and the combined predictive value of the three indexes was better than that of the single index.
