【摘要】：目的：探討不同子宮手術方式對生育期婦女卵巢功能的影響。方法：選取因生殖系統(tǒng)疾病行手術治療的180例生育期女性患者,檢測術前、術后6個月的血FSH、E2、AMH,并評估其潮熱等更年期癥狀。結果：術前和術后6個月患者均未出現(xiàn)卵巢功能衰竭。與術前比較,年齡40歲者術后6個月的AMH顯著下降(P0.05)。子宮肌瘤剔除術的患者術前AMH為(3.54±1.31) ng/ml,術后(3.69±1.35) ng/ml,差異無統(tǒng)計學意義(P=0.612)。與術前比較,子宮次全切除術患者術后AMH降低,但差異無統(tǒng)計學意義；子宮全切術(包括筋膜外子宮全切術)患者的術后AMH顯著降低[(1.45±0.68) ng/ml vs (1.80±0.96) ng/ml, P0.05];廣泛性子宮切除術患者的術后AMH顯著降低[(2.72±1.08) ng/ml vs (3.31±0.92) ng/ml,P0.05];腹腔鏡下子宮切除術患者的術后AMH顯著降低[(1.56±1.00) ng/ml vs (1.94±1.10) ng/ml,P0.05]。結論：子宮手術短期內不會導致卵巢功能衰竭,年齡、子宮切除方式與途徑可能是子宮手術后短期內卵巢儲備功能下降的影響因素。
[Abstract]:Objective: to investigate the effect of different methods of uterine operation on ovarian function of women in reproductive period. Methods: 180 women with reproductive system diseases were selected and examined before operation. 6 months after operation, serum FSHE 2 AMH was measured, and menopausal symptoms such as hot flashes were evaluated. Results: there was no ovarian failure before and 6 months after operation. The AMH of the patients with hysteromyomectomy was 3.54 鹵1.31) ng / ml before operation, and 3.69 鹵1.35) ng / ml after operation. There was no significant difference in AMH between the patients with subtotal hysterectomy and the patients with subtotal hysterectomy before operation, but there was no significant difference between the patients with subtotal hysterectomy and the patients with subtotal hysterectomy. Postoperative AMH in patients with total hysterectomy (including total hysterectomy) was significantly lower [1.45 鹵0.68) ng/ml vs 1.80 鹵0.96) ng / ml, P0.05]; postoperative AMH in patients with extensive hysterectomy was significantly lower [2.72 鹵1.08) ng/ml vs 3.31 鹵0.92) ng / ml P 0.05]; postoperative AMH in patients with laparoscopic hysterectomy was significantly lower than that in patients undergoing laparoscopic hysterectomy. AMH was significantly decreased [1.56 鹵1.00) ng/ml vs 1.94 鹵1.10) ng / ml (P0.05). Conclusion: ovarian failure will not occur in the short term after uterine surgery. Age, hysterectomy mode and pathway may be the influencing factors for the decline of ovarian reserve function in the short term after hysterectomy.
