【摘要】：目的探討maspin和c-erbB-2在妊娠滋養(yǎng)細(xì)胞疾病中的表達(dá)及臨床意義。方法收集225例葡萄胎患者,隨訪3~27個(gè)月,27例發(fā)展為侵蝕性葡萄胎,12例發(fā)展為絨毛膜癌。采用免疫組化PV9000二步法檢測(cè)24例正常早期妊娠(對(duì)照組)、49例葡萄胎(葡萄胎組)、39例妊娠滋養(yǎng)細(xì)胞腫瘤(侵葡+絨癌組)maspin和c-erbB-2的表達(dá)。結(jié)果葡萄胎組maspin陽(yáng)性表達(dá)率(65.3%,32/49)與對(duì)照組(91.7%,22/24)及侵葡+絨癌組(35.9%,14/39)比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。hCG105 U/L、子宮體積孕周及卵巢黃素囊腫直徑6cm組maspin陽(yáng)性率分別為34.6%(18/52)、37.0%(20/54)和36.6%(15/41),均低于無(wú)上述高危因素組(P0.05)。葡萄胎組c-erbB-2陽(yáng)性表達(dá)率(53.1%,26/49)與侵葡+絨癌組(74.4%,29/39)比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。子宮體積孕周組c-erbB-2陽(yáng)性表達(dá)率(74.1%,40/54)與子宮體積≤孕周組(44.1%,15/34)比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。葡萄胎組maspin與c-erbB-2的表達(dá)呈負(fù)相關(guān)(r=-0.270,P0.05)。侵葡+絨癌組FIGO預(yù)后評(píng)分≥7分組maspin表達(dá)較7分組明顯下降(P0.05)。結(jié)論maspin和c-erbB-2可作為預(yù)測(cè)葡萄胎惡變的參考指標(biāo),maspin的檢測(cè)可能對(duì)妊娠滋養(yǎng)細(xì)胞疾病預(yù)后評(píng)估有一定的參考意義。
[Abstract]:Objective to investigate the expression and clinical significance of maspin and c-erbB-2 in gestational trophoblastic diseases. 27 cases of invasive hydatidiform mole were followed up for 3 ~ 27 months. Immunohistochemical PV9000 two-step method was used to detect 24 cases of normal early pregnancy (control group 49 cases of hydatidiform mole) (hydatidiform mole group 39 cases of gestational trophoblastic tumor). (expression of maspin and c-erbB-2 in invasive staphylococcal choriocarcinoma group. The positive rates of maspin in gestational weeks of uterine volume and in 6cm diameter of ovarian flavin cysts were 34.6 / 1852% and 37.0% 20 / 54, respectively, and 36.6% of 41 cases were lower than those of non-high risk factors group (P0.055.The positive expression rate of c-erbB-2 in hydatidiform mole group was 53.11% 2649), compared with that in invasive staphylochoriocarcinoma group (74.440 / 2939).
