【摘要】：目的探討外陰富于細(xì)胞性血管纖維瘤(CAF)的臨床病理特征、鑒別診斷以及診治方法和預(yù)后。方法選擇2014-11-04中國(guó)醫(yī)科大學(xué)附屬盛京醫(yī)院收治的1例外陰富于細(xì)胞性血管纖維瘤的病例,進(jìn)行臨床特點(diǎn)、組織形態(tài)學(xué)、免疫組織化學(xué)研究。結(jié)合文獻(xiàn)分析本病的臨床表現(xiàn)、病理形態(tài)特點(diǎn)及鑒別診斷。結(jié)果富于細(xì)胞性血管纖維瘤呈不規(guī)則小葉狀,瘤細(xì)胞呈短梭形、大小一致、核卵圓至梭形、核分裂象不易見。腫瘤含大量的小至中等大小的血管,以無分支的毛細(xì)血管為主。病理切片特點(diǎn)為疏松排列的梭形細(xì)胞、脂肪瘤樣細(xì)胞和突出的厚壁血管。免疫組織化學(xué):腫瘤細(xì)胞Vimentin陽性,CD34陽性。CD31、SMA、S-100、HMB-45均陰性。結(jié)論富于細(xì)胞性血管纖維瘤是一種極少見的特殊的外陰部良性軟組織腫瘤,腫瘤可能存在間質(zhì)細(xì)胞增生活性不同的組織學(xué)亞型。治療以手術(shù)為主,術(shù)后需嚴(yán)密復(fù)查。
[Abstract]:Objective to investigate the clinicopathological features, differential diagnosis, diagnosis and treatment and prognosis of vulvar hemangiofibroma (CAF). Methods one case of cytosolic angiofibroma was selected from Shengjing Hospital affiliated to China Medical University from 2014-11-04. The clinical features, histomorphology and immunohistochemistry were studied. The clinical manifestations, histopathologic features and differential diagnosis of the disease were analyzed. Results Cytoangiofibroma presented irregular lobular shape and short fusiform tumor cells. The tumor contains a large number of small to medium sized blood vessels, mainly non-branched capillaries. The pathological sections are characterized by loosely arranged spindle cells. Immunohistochemistry: tumor cells were Vimentin positive and CD34 positive. CD31 SMAS-100 HMB-45 was negative. Conclusion Cytoangiofibroma is a rare and special benign soft tissue tumor of the vulva. The tumor may have histological subtypes with different stromal cell proliferation activity.
