【摘要】：目的探討孕婦牙周狀況與早產及低體質量兒的關系。方法收集出現先兆早產的孕婦,選擇其中出現早產或低體質量新生兒的孕婦50例為早產組,以同期來醫(yī)院足月待產的孕婦50例為足月產組。所有產婦分娩前均進行全口牙周檢查,記錄菌斑指數(PLI)、牙石指數(CI)、牙齦出血指數(BI)、牙周探診深度(PD)及臨床附著喪失(CAL)。檢測2組孕婦外周血中炎癥相關細胞因子腫瘤壞死因子α、前列腺素E2、基質金屬蛋白酶-9和C反應蛋白的水平。將分娩孕周及新生兒體質量、牙周狀況和外周血細胞因子水平分別做相關性分析。結果與足月產組比較,早產組孕婦的牙周狀況較差,PLI,BI,CI及牙周炎患病率均顯著升高(P0.05);孕婦的PLI,BI,CI及牙周炎患病率與分娩孕周及新生兒體質量呈負相關。早產組外周血中炎癥相關細胞因子水平均顯著高于足月產組;孕婦外周血中細胞因子水平與PLI,BI,CI和牙周炎患病率呈正相關,與分娩孕周及新生兒體質量呈負相關。結論孕婦牙周狀況與早產低體質量新生兒間可能存在一定的相關性;牙周狀況對早產及低體質量兒的影響可能與外周血中炎癥相關細胞因子的改變有關。
[Abstract]:Objective to investigate the relationship between periodontal status of pregnant women and premature delivery and low body weight infants. Methods 50 pregnant women with premature birth or low body weight newborn were selected as premature delivery group. A total of 50 pregnant women with full term labor in the hospital during the same period were selected as the term delivery group. All the pregnant women underwent periodontal examination before delivery. The plaque index, dental stone index, gingival bleeding index, periodontal probing depth and clinical attachment loss were recorded. The levels of inflammatory cytokines tumor necrosis factor 偽, prostaglandin E 2, matrix metalloprotein in peripheral blood of pregnant women in both groups were measured. Levels of enzyme -9 and C-reactive protein. The correlation between periodontal status and peripheral blood cytokine levels was analyzed. The prevalence of PLII-BICI and periodontitis in preterm pregnant women was significantly higher than that in pregnant women (P 0.05), and the prevalence of PLII-BICI and periodontitis in pregnant women was negatively correlated with maternal gestational weeks and neonatal body mass. Inflammatory cytokines in peripheral blood of preterm labor group. The average value was significantly higher than that in term group. The levels of cytokines in maternal peripheral blood were positively correlated with the prevalence of PLII-BICI and periodontitis, and negatively correlated with maternal gestational weeks and neonatal body mass. Conclusion there may be a certain correlation between maternal periodontal status and preterm low birth weight newborns. The effect of periodontal status on premature delivery and low body mass infants may be related to the changes of inflammatory cytokines in peripheral blood.
