【摘要】：目的:本課題通過(guò)觀察活血通管湯聯(lián)合介入再通術(shù)治療血瘀型輸卵管阻塞性不孕的臨床療效,分析綜合治療后宮內(nèi)妊娠率、輸卵管再通率以及中醫(yī)癥狀及體征的改善,探討其治療的作用機(jī)理,以驗(yàn)證活血通管湯聯(lián)合介入再通術(shù)治療本病的療效。希望為輸卵管阻塞性不孕患者尋找最佳的治療方法。方法:本課題選用就診于河南中醫(yī)藥大學(xué)第二附屬醫(yī)院婦產(chǎn)科門(mén)診,符合納入標(biāo)準(zhǔn)的血瘀型輸卵管阻塞性不孕患者60例。采用隨機(jī)數(shù)字表法將研究對(duì)象分為治療組和對(duì)照組。兩組患者均已行輸卵管介入再通術(shù)。治療組于術(shù)后第2天開(kāi)始服用活血通管湯,由河南中醫(yī)藥大學(xué)第二附屬醫(yī)院制劑中心統(tǒng)一熬制成200ml袋裝,一付藥煎成兩袋,每日一劑,分早晚溫服。對(duì)照組于術(shù)后第2天開(kāi)始服用丹桃合劑,早、中、晚于飯后半小時(shí)口服,每次20ml。兩組患者從次月開(kāi)始,于月經(jīng)干凈后3天開(kāi)始服藥,連用15天。1個(gè)月經(jīng)周期連續(xù)服藥15天為1個(gè)療程。次月可以試孕。受孕者停藥,未孕者連用6個(gè)療程。結(jié)果:治療組經(jīng)治療后宮內(nèi)妊娠率為56.7%;輸卵管通暢的總有效率達(dá)到93.3%,痊愈率為80%;盆腔炎相關(guān)癥狀臨床痊愈率為53.3%,總有效率為90%。對(duì)照組經(jīng)治療后宮內(nèi)妊娠率為23.3%;輸卵管通暢的總有效率達(dá)到80%,痊愈率為33.3%;盆腔炎相關(guān)癥狀臨床痊愈率23.3%,有效率73.3%。比較兩組治療后總有效率及痊愈率,P0.05,有顯著性差異,說(shuō)明治療組治療血瘀型輸卵管阻塞性不孕比對(duì)照組治療效果好,治愈率高。治療后治療組在癥狀和體征所表現(xiàn)出來(lái)的臨床癥狀方面,與對(duì)照組相比臨床療效顯著,P0.05,具有顯著性差異。結(jié)論:活血通管湯聯(lián)合介入再通術(shù)治療血瘀型輸卵管阻塞性不孕,具有提高宮內(nèi)妊娠率,降低輸卵管再閉塞率,緩解盆腔炎臨床癥狀,臨床療效顯著。充分體現(xiàn)了此綜合療法在治療近端輸卵管阻塞性不孕癥方面的優(yōu)勢(shì),具有較高的臨床研究?jī)r(jià)值,值得進(jìn)一步深入探討與研究。
[Abstract]:Objective: to observe the clinical effect of Huoxue Tongtuan decoction combined with interventional recanalization in the treatment of obstructive infertility of fallopian tube with blood stasis, and to analyze the improvement of intrauterine pregnancy rate, fallopian tube recanalization rate and the symptoms and signs of traditional Chinese medicine after comprehensive treatment. To explore the mechanism of its treatment, In order to verify the curative effect of Huoxue Tongguan decoction combined with interventional recanalization for the treatment of this disease, we hope to find the best treatment method for the infertile patients with fallopian tube obstruction. Methods: this subject was selected to visit the outpatient department of gynecology and obstetrics of the second affiliated Hospital of Henan University of traditional Chinese Medicine. Sixty cases of oviduct obstructive infertility with blood stasis type were divided into treatment group and control group by random digital table method. The patients in both groups had undergone fallopian tube interventional recanalization. Days began to take Huoxue Tongguo decoction, Prepared by the preparation Center of the second affiliated Hospital of Henan University of traditional Chinese Medicine, 200 ml bags were made, one drug was fried into two bags, one dose was taken in the morning and evening, and the control group began to take Dantao mixture on the second day after operation, morning and middle. Take orally later than half an hour after meal, each time 20ml. Two groups of patients begin from the following month, begin to take medicine 3 days after menstruation clean, use continuously 15 days. 1 menstrual cycle continuously take medicine for 15 days as a course of treatment. The next month can test pregnancy. Results: after treatment, the rate of intrauterine pregnancy was 56.7 in the treatment group; the total effective rate of tubal patency was 93.3and the cure rate was 800.The clinical recovery rate of pelvic inflammation related symptoms was 53.3, and the total effective rate was 900.The control group was treated with treatment. The total effective rate of tubal patency was 80, the cure rate was 33.30.The clinical recovery rate of pelvic inflammation related symptoms was 23.3cm, the effective rate was 73.3%. The total effective rate and cure rate (P0.05) were significantly different between the two groups. The results showed that the treatment group had better therapeutic effect and higher cure rate than the control group in the treatment of oviduct obstructive infertility with blood stasis. The clinical symptoms and signs of the treatment group after treatment were better than those in the control group. Conclusion: Huoxue Tongguan decoction combined with interventional recanalization can improve the rate of intrauterine pregnancy and decrease the rate of reocclusion of fallopian tube. The clinical symptoms of pelvic inflammatory disease are alleviated and the clinical effect is remarkable. It fully reflects the advantages of this comprehensive therapy in the treatment of proximal tubal obstructive infertility. It has a high clinical value and is worthy of further discussion and study.
