【摘要】：目的：比較陰道液胎盤α1-微球蛋白（PAMG-1），人胰島素樣生長因子結合蛋白1（IGFBP-1）檢測法及PH測定法診斷胎膜早破（PROM）的準確性。 方法：120位孕齡介于11-42周懷疑有胎膜早破癥狀/體征的孕婦被納入實驗，并分別用胎盤α1-微球蛋白免疫層析檢測法，人胰島素樣生長因子結合蛋白1免疫層析檢測法，，及PH測定法進行診斷。 結果：在120例孕婦中，PAMG-1免疫層析檢測法，IGFBP-1免疫層析檢測法，及PH測定法的靈敏度，特異度，陽性預測值及陰性預測值分別為100%,100%,100%,及100%,93.33%,98.89%,96.55%及97.80%,93.33%,94.44%,84.85%,及97.7%。PAMG-1測定法與PH測定法相比，陽性符合率為84.85%，陰性符合率為97.70%，總體符合率為94.17%，kappa值為0.85。PAMG-1測定法與IGFBP-1測定法相比，陽性符合率，陰性符合率及總體符合率分別為96.55%，97.80%及97.50%，kappa值為0.93。 結論：陰道液胎盤α1-微球蛋白免疫層析檢測法是三種方法中診斷胎膜早破最靈敏的方法，具有更高的靈敏度，特異度，陽性預測值及陰性預測值。
[Abstract]:Objective: to compare the placenta 偽 1-microglobulin (PAMG-1) of vaginal fluid. The accuracy of human insulin-like growth factor binding protein 1 IGFBP-1 assay and PH assay in diagnosis of premature rupture of membranes. Methods one hundred and twenty pregnant women aged between 11-42 weeks suspected of premature rupture of membranes were included in the experiment, and the placental 偽 1-microglobulin immunochromatography was used to detect the symptoms / signs of premature rupture of membranes. Human insulin-like growth factor binding protein 1 was diagnosed by immunochromatography and PH assay. Results: the sensitivity and specificity of IGFBP-1 immunochromatographic assay and PH assay were analyzed in 120 pregnant women. The positive predictive value and the negative predictive value were 100 / 100 and 93.33 / 100, respectively. The positive predictive value and the negative predictive value were 98.899.55% and 97.80,93.33% respectively. The positive coincidence rate was 84.85 and the negative coincidence rate was 97.70%. Compared with PH method, the positive coincidence rate was 84.85 and the negative coincidence rate was 97.70% by 97.7. PAMG-1 method. The overall coincidence rate was 94.17kappa, 0.85.PAMG-1, and the positive coincidence rate was higher than that of IGFBP-1. The negative coincidence rate and the overall coincidence rate were 96.55% and 97.50%, respectively. Conclusion: the detection of 偽 1-microglobulin immunochromatography in placenta of vaginal fluid is the most sensitive method for the diagnosis of premature rupture of membranes. It has higher sensitivity, specificity, positive predictive value and negative predictive value.
