【摘要】：卵胞質(zhì)內(nèi)精子注射(intracytoplasmic sperm injection,ICSI)是目前治療男性不育最有效的方法,但在ICSI周期中仍然有1%~3%的受精失敗發(fā)生。近年來(lái)逐漸發(fā)展起來(lái)的卵母細(xì)胞輔助激活(assisted oocyte activation,AOA)技術(shù)在改善ICSI后受精失敗患者的妊娠結(jié)局方面取得了較為明顯的效果,有望成為治療男性不育的重要輔助手段。本文綜述了卵母細(xì)胞激活機(jī)制領(lǐng)域的研究成果,揭示了精子中的PLCξ蛋白引起卵母細(xì)胞內(nèi)Ca2+震蕩進(jìn)而觸發(fā)激活的機(jī)制,介紹了目前對(duì)ICSI受精失敗患者精子激活能力的檢測(cè)與診斷方法,以及不同卵母細(xì)胞輔助激活技術(shù)如電激活,Ca2+載體和Sr Cl2等激活技術(shù)的研究現(xiàn)狀,并總結(jié)了這些技術(shù)在安全性方面所獲得的證據(jù),在此基礎(chǔ)上對(duì)AOA技術(shù)今后的發(fā)展方向和前景作了探討。
[Abstract]:Intracytoplasmic sperm injection (ICSI) is the most effective method in the treatment of male infertility. However, in the ICSI cycle, there are still 1 / 3% of fertilization failures. The oocyte assisted activation (oocyte) has been developed in recent years. Assisted oocyte activation. AOA-based technique has achieved significant results in improving the pregnancy outcome of patients who failed to fertilize after ICSI. It is expected to be an important auxiliary method in the treatment of male infertility. This article reviews the research achievements in the field of oocyte activation mechanism. The mechanism of Ca2 oscillation in oocytes induced by PLC 尉 protein in spermatozoa was revealed, and the methods of detection and diagnosis of spermatozoa activation ability in patients with failed ICSI fertilization were introduced. The research status of different oocyte assisted activation techniques such as electroactivation of Ca 2 carrier and Sr Cl2 were also reviewed, and the evidence of safety of these techniques was summarized. On this basis, the future development direction and prospect of AOA technology are discussed.
