【摘要】：目的經(jīng)會陰盆底三維超聲動態(tài)觀察便秘產(chǎn)婦的肛提肌裂孔解剖形態(tài)重塑及功能改變。方法選取2014年7月至2017年6月在我院婦產(chǎn)科定期行產(chǎn)后復(fù)查的女性110例(產(chǎn)后組),其中有便秘癥狀45例(便秘產(chǎn)婦),無便秘癥狀65例(無便秘產(chǎn)婦),同期選取未育的無便秘癥狀健康婦女100例(對照組)。所有婦女均進(jìn)行經(jīng)會陰盆底三維超聲檢查,觀察并測量比較各組婦女的盆底解剖形態(tài)結(jié)構(gòu)及特征,測量各組婦女在靜息狀態(tài)、縮肛動作和Valsalva動作3種狀態(tài)下的肛提肌裂孔面積、周長及肛提肌厚度。結(jié)果產(chǎn)后婦女盆底組織結(jié)構(gòu)疏松,對照組婦女肛提肌裂孔結(jié)構(gòu)完整緊湊。在靜息狀態(tài)、縮肛動作及Valsalva動作3種狀態(tài)下,產(chǎn)后組婦女的肛提肌裂孔面積、周長及肛提肌厚度均明顯大于對照組婦女,差異均有統(tǒng)計學(xué)意義(P0.05)。產(chǎn)后組便秘產(chǎn)婦在3種狀態(tài)下的肛提肌裂孔面積及周長均明顯大于無便秘產(chǎn)婦(P0.05);在縮肛動作時便秘產(chǎn)婦的肛提肌厚度明顯薄于無便秘產(chǎn)婦(P0.05),在靜息狀態(tài)及Valsalva動作時,便秘產(chǎn)婦與無便秘產(chǎn)婦的肛提肌厚度比較,差異均無統(tǒng)計學(xué)意義(P0.05)。結(jié)論產(chǎn)后婦女盆底解剖結(jié)構(gòu)發(fā)生重塑,經(jīng)會陰三維超聲能有效觀察產(chǎn)后女性尤其便秘產(chǎn)婦盆底解剖結(jié)構(gòu)與功能的變化。
[Abstract]:Objective to observe the anatomical morphology and functional changes of anal levator muscle foramen in constipation parturients by three-dimensional ultrasound at perineal floor. Methods from July 2014 to June 2017, 110 women (postpartum group) who were regularly reexamined after delivery in obstetrics and gynecology department of our hospital were selected, including 45 women with constipation symptoms (constipation parturients), 65 women without constipation symptoms (no constipation parturients), and 100 infertile healthy women without constipation symptoms (control group). All women underwent three-dimensional ultrasound examination of perineal floor, observed and compared the anatomical structure and characteristics of pelvic floor of women in each group, and measured the foramen area, circumference and thickness of levator anal muscle in resting state, anal contraction action and Valsalva action. Results the pelvic floor tissue structure of postpartum women was loose, and the structure of anal levator muscle foramen in control group was complete and compact. In resting state, anal contraction action and Valsalva action, the foramen area, circumference and thickness of levator anal muscle in postpartum group were significantly higher than those in control group (P 0.05), and the area, circumference and thickness of levator anal muscle in postpartum group were significantly higher than those in control group (P 0.05). The area and circumference of anal levator muscle in postpartum group were significantly larger than those in non-constipation parturients (P 0.05); the thickness of anal levator muscle in constipation parturients was significantly thinner than that in non-constipation parturients during anal contraction (P 0.05). In resting state and Valsalva action, there was no significant difference in anal levator muscle thickness between constipation parturients and non-constipation parturients (P 0.05). Conclusion the anatomical structure of pelvic floor of postpartum women is reshaped. Three-dimensional ultrasound of perineum can effectively observe the changes of anatomical structure and function of pelvic floor of postpartum women, especially those with constipation.
