【摘要】：目的:探討實(shí)時(shí)剪切波彈性成像(SWE)技術(shù)對TI-RADS 4級甲狀腺單發(fā)實(shí)性結(jié)節(jié)良惡性判定的應(yīng)用價(jià)值。方法:對132例TI-RADS 4級甲狀腺單發(fā)實(shí)性結(jié)節(jié)患者行SWE檢查,獲取病灶楊氏模量平均值(Emean),以病理結(jié)果為金標(biāo)準(zhǔn)繪制ROC曲線,得出最佳界值。將4a級結(jié)節(jié)中Emean低于界值者判定為3級,將4b級結(jié)節(jié)中Emean高于界值者判定為5級;余保持不變。分析調(diào)整前后的診斷結(jié)果。結(jié)果:常規(guī)超聲檢查判定為4a級結(jié)節(jié)73例,根據(jù)SWE,51例被判定為3級結(jié)節(jié),其中良性46例,惡性5例;常規(guī)超聲檢查判定為4b級結(jié)節(jié)59例,根據(jù)SWE,42例被判定為5級結(jié)節(jié),其中良性4例,惡性38例。結(jié)論:對于TI-RADS 4級結(jié)節(jié)行SWE檢查可以進(jìn)一步判定良、惡性,有效降低穿刺活檢率。
[Abstract]:Objective: to evaluate the value of real time shear wave elastography (SWE) in the diagnosis of benign and malignant solitary solid thyroid nodules of TI-RADS grade 4. Methods: 132 cases of TI-RADS grade 4 thyroid single solid nodules were examined by SWE. The average value of Young's modulus (Emean),) of the lesions was obtained. The ROC curve was drawn according to the pathological results as gold standard, and the best boundary value was obtained. The Emean of 4a grade nodules is lower than the boundary value, and that of 4b nodules is lower than the boundary value. The Emean of 4b nodules is rated as grade 5, while the rest remains unchanged. Analyze the diagnostic results before and after adjustment. Results: according to SWE,51, 73 cases were diagnosed as 4a grade nodules, 46 cases as benign nodules, 5 cases as malignant nodules, 59 cases as grade 4b nodules and 59 cases as grade 5 nodules according to SWE,42 cases. Among them, benign 4 cases, malignant 38 cases. Conclusion: SWE examination for TI-RADS grade 4 nodules can further determine benign and malignant lesions and reduce the biopsy rate effectively.
