【摘要】：目的探討應用組織多普勒成像技術(shù)(TDI)評價擴張型心肌病(DCM)患者右室功能的臨床價值。方法 DCM患者84例,根據(jù)心室的大小,分為左心型、右心型及全心型三組。健康對照組50例,取標準心尖四腔心切面,應用TDI定量分析三尖瓣前葉及隔葉瓣環(huán)處的組織運動速度。結(jié)果與正常組比較,三組DCM患者三尖瓣環(huán)處組織運動VS、VE、VE/VA均明顯減低,差異有統(tǒng)計學意義(P0.05),并以全心型組最為顯著。三組DCM患者間兩兩比較,全心型后間隔瓣環(huán)處VS高于右心型,左心型側(cè)壁瓣環(huán)處VE/VA與全心型比較差異無統(tǒng)計學意義(P0.05),余VS、VE、VE/VA表現(xiàn)為全心型右心型左心型,差異有統(tǒng)計學意義(P0.05)。結(jié)論 TDI速度指標VS能快速評價DCM患者右室收縮功能,而VE、VE/VA能快速評價DCM患者右室舒張功能,具有重要的臨床價值。
[Abstract]:Objective to evaluate the clinical value of tissue Doppler imaging (TDI) in evaluating right ventricular function in patients with dilated cardiomyopathy (DCM). Methods 84 patients with DCM were divided into three groups according to ventricular size: left heart type, right heart type and whole heart type. TDI was used to analyze the tissue motion velocity of anterior and septal annulus of tricuspid valve in 50 cases of healthy control group. Results compared with the normal group, the VS,VE,VE/VA of tricuspid annulus was significantly decreased in the three groups of DCM patients (P0.05), especially in the whole heart type group. The VS at the posterior septal annulus of the whole heart type was higher than that of the right heart type (P0.05), while the VE/VA at the left ventricular type was not significantly different from that of the whole heart type (P0.05), while the remaining VS,VE,VE/VA showed the left heart type of the right heart type (P0.05). Conclusion TDI velocity index VS can quickly evaluate right ventricular systolic function in DCM patients, while VE,VE/VA can quickly evaluate right ventricular diastolic function in DCM patients, which has important clinical value.
