【摘要】：目的探討二維斑點追蹤超聲心動圖在老年心力衰竭患者診斷中的應用價值。方法 2012年10月至2014年10月在該院就診的老年心力衰竭患者49例設為觀察組。同期在該院接受治療的非心力衰竭的老年患者49例設為對照組。通過實時三維超聲心動圖測量兩組被檢人員的左心房主動射血分數(shù)(LAAEF)、左心房被動射血分數(shù)(LAPEF)、左心房整體射血分數(shù)(LAEF)、左心房最小容積(LAVmin)、左心房最大容積(LAVmax)以及左心房收縮前容積(LAVpre)水平,并且應用二維斑點超聲技術測定各房壁的左心室舒張早期峰值平均應變率(m SRe)、舒張晚期峰值平均應變率(m SRa)以及收縮期峰值平均應變率(m SRs)水平。結果觀察組LAAEF、LAPEF、LAEF都低于對照組(P0.01)。觀察組LAVmin、LAVmax、LAVpre都高于對照組(P0.01)。較對照組而言,觀察組的m SRs、m SRe下降,m SRa上升(P0.01)。結論老年心力衰竭患者的左心房功能均存在不同程度的下降,二維斑點追蹤超聲心動圖能夠及時準確地反映老年心力衰竭患者左心房功能發(fā)生的變化,從而有利于早期展開治療。
[Abstract]:Objective to evaluate the value of two-dimensional dot-tracing echocardiography in the diagnosis of elderly patients with heart failure. Methods from October 2012 to October 2014, 49 elderly patients with heart failure were treated as observation group. At the same time, 49 elderly patients with non-heart failure received treatment in the hospital as control group. Left atrial active ejection fraction (LAAEF), left atrial passive ejection fraction (LAPEF), left atrial global ejection fraction (LAEF), left atrial minimum volume (LAVmin), left atrial maximum volume (LAVmax) and left heart were measured by real-time three-dimensional echocardiography. Atrial presystolic volume (LAVpre) level, The mean peak strain rate (mSRe), peak strain rate (mSRa) and peak systolic strain rate (mSRs) of left ventricle were measured by two-dimensional speckle ultrasound. Results the LAAEFA LAPEFL LAEF in the observation group was lower than that in the control group (P 0.01). Both LAVmin and LAVmaxLAVpre in the observation group were higher than those in the control group (P0.01). Compared with the control group, the mSRsm SRe decreased and mSRa increased in the observation group (P0.01). Conclusion the left atrial function of elderly patients with heart failure is decreased to some extent. Two-dimensional dot-tracing echocardiography can accurately reflect the changes of left atrial function in elderly patients with heart failure. Thus, it is beneficial to the early development of treatment.
