【摘要】：目的:探討超聲在髖關(guān)節(jié)前上盂唇撕裂中的診斷價值。方法:連續(xù)收集102例臨床擬診髖關(guān)節(jié)盂唇撕裂患者的超聲檢查、磁共振檢查(magnetic resonance imaging,MRI)和關(guān)節(jié)鏡手術(shù)資料,其中男性42例,女性60例,平均年齡(35.14±9.16)歲(13~60歲),左側(cè)髖關(guān)節(jié)44例,右側(cè)髖關(guān)節(jié)58例。以關(guān)節(jié)鏡手術(shù)為金標(biāo)準(zhǔn),評估超聲診斷髖關(guān)節(jié)前上盂唇撕裂的敏感性、特異性、陽性預(yù)測值、陰性預(yù)測值和準(zhǔn)確性,并與MRI比較。結(jié)果:所有病例均經(jīng)關(guān)節(jié)鏡手術(shù)證實,其中91例為髖關(guān)節(jié)前上盂唇撕裂,11例髖關(guān)節(jié)盂唇無撕裂。超聲正確診斷69例,包括髖關(guān)節(jié)前上盂唇撕裂60例和髖關(guān)節(jié)盂唇無撕裂9例;其余33例中,超聲誤診2例,漏診31例;超聲評估髖關(guān)節(jié)前上盂唇撕裂的敏感性、特異性、陽性預(yù)測值、陰性預(yù)測值和準(zhǔn)確性分別為65.93%、81.82%、96.77%、22.50%和67.65%。MRI正確診斷77例,包括髖關(guān)節(jié)前上盂唇撕裂70例和髖關(guān)節(jié)盂唇無撕裂7例;其余的25例中,誤診4例,漏診21例;MRI評估髖關(guān)節(jié)前上盂唇撕裂的敏感性、特異性、陽性預(yù)測值、陰性預(yù)測值和準(zhǔn)確性分別為76.92%、63.64%、94.59%、25.00%和75.49%。超聲檢查與MRI檢查結(jié)果相比,在評估髖關(guān)節(jié)前上盂唇撕裂的準(zhǔn)確性上,超聲檢查低于MRI,差異有統(tǒng)計學(xué)意義(P0.01)。結(jié)論:超聲診斷髖關(guān)節(jié)前上盂唇撕裂的敏感性略低于MRI,但超聲檢查的特異性高,動態(tài)評估是超聲檢查的優(yōu)勢,可以作為一種有效的檢查方法。
[Abstract]:Objective: to evaluate the diagnostic value of ultrasound in anterior upper pelvis lip tear of hip joint. Methods: the clinical data of 102 patients with clinically diagnosed pelvic lip tears were collected, including 42 males and 60 females, with an average age of 35.14 鹵9.16 years, and 44 patients with left hip joint. 58 cases of right hip joint. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of ultrasonography in the diagnosis of anterior upper pelvis lip tear of hip joint were evaluated using arthroscopic surgery as the golden standard, and compared with MRI. Results: all the cases were confirmed by arthroscopy, of which 91 cases were laceration of anterior upper pelvis lip of hip joint, 11 cases had no tear of labium pelvis of hip joint. 69 cases were correctly diagnosed by ultrasound, including 60 cases of anterior upper pelvic lip tear and 9 cases of non-laceration of the labium of the hip, 2 cases were misdiagnosed and 31 cases missed the diagnosis by ultrasound in the remaining 33 cases, the sensitivity and specificity of ultrasound in evaluating the tear of anterior upper pelvic lip of hip joint were evaluated. The positive predictive value, the negative predictive value and the accuracy of the positive predictive value and the negative predictive value were 65.93%, 81.82% and 22.50%, respectively, and 77 cases were correctly diagnosed by 67.65%.MRI, including 70 cases of anterior upper pelvic lip tear and 7 cases of no laceration of the labium pelvis of the hip, the other 25 cases were misdiagnosed in 4 cases. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of MRI in 21 cases with missing diagnosis were 76.92 ~ 63.64 ~ 94.599.59 ~ 25.00% and 75.4995% respectively. Compared with the results of MRI, the accuracy of ultrasonography in evaluating anterior upper labial laceration of hip joint was lower than that of MRI, and the difference was statistically significant (P 0.01). Conclusion: the sensitivity of ultrasonography in the diagnosis of anterior upper pelvic lip tear of hip joint is slightly lower than that of MRI, but the specificity of ultrasonic examination is high. Dynamic evaluation is the advantage of ultrasonography and can be used as an effective examination method.
