【摘要】：目的：對(duì)三陰性乳腺癌及HER-2過(guò)表達(dá)型乳腺癌的病理、磁共振動(dòng)態(tài)增強(qiáng)特征及ADC值進(jìn)行探討及分析,提高對(duì)兩者病理、磁共振特征的認(rèn)識(shí)和判斷。方法：對(duì)經(jīng)免疫組化證實(shí)為三陰性乳腺癌的68名患者及55例HER-2過(guò)表達(dá)型乳腺癌患者的病理、MR動(dòng)態(tài)增強(qiáng)特征進(jìn)行分析,測(cè)量ADC值,同時(shí)進(jìn)行統(tǒng)計(jì)學(xué)比較。結(jié)果：68例TNBC中浸潤(rùn)性導(dǎo)管癌59例,髓樣癌5例,55例HER-2過(guò)表達(dá)型乳腺癌中浸潤(rùn)性導(dǎo)管癌39例,導(dǎo)管原位癌13例,兩組乳腺癌在浸潤(rùn)性導(dǎo)管癌、髓樣癌、導(dǎo)管原位癌比較有差異。TNBC在MRI上大多表現(xiàn)為類(lèi)圓形、邊緣光整、環(huán)狀強(qiáng)化及相對(duì)高的ADC值,HER-2過(guò)表達(dá)性乳腺癌在MRI上大多表現(xiàn)為不規(guī)則形、邊緣毛刺、不均勻強(qiáng)化,相對(duì)低的ADC值。而在患者年齡,是否腫塊及TIC上,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論：TNBC多為浸潤(rùn)性導(dǎo)管癌及髓樣癌,HER-2過(guò)表達(dá)型乳腺癌多為浸潤(rùn)性導(dǎo)管癌及導(dǎo)管原位癌,乳腺M(fèi)R1動(dòng)態(tài)增強(qiáng)形態(tài)學(xué)特征中病灶的形狀、邊緣、強(qiáng)化方式及ADC值對(duì)診斷TNBC及HER-2過(guò)表達(dá)性乳腺癌具有一定參考價(jià)值。
[Abstract]:Objective: to study and analyze the pathology, dynamic enhancement characteristics and ADC value of triple negative breast cancer and HER-2 overexpression breast cancer. Methods: dynamic contrast-enhanced MRI features of 68 patients with triple-negative breast cancer and 55 patients with HER-2 overexpression were analyzed and ADC values were measured. Results among 68 cases of TNBC, 59 cases of invasive ductal carcinoma, 5 cases of medullary carcinoma and 55 cases of HER-2 overexpression breast cancer, 39 cases of invasive ductal carcinoma, 13 cases of ductal carcinoma in situ, two groups of breast cancer in invasive ductal carcinoma, medullary carcinoma. Ductal carcinoma in situ most of the breast cancer on MRI appeared round, edge smooth, circular enhancement and relatively high ADC value of HER-2 over-expression breast cancer on MRI most of the irregular shape, edge burr, uneven enhancement. There was no significant difference in age, mass and TIC in patients with ADC. Conclusion the majority of invasive ductal carcinoma and medullary carcinoma are infiltrative ductal carcinoma and breast cancer with HER-2 overexpression in situ. The shape, edge, enhancement mode and ADC value of the lesions in the dynamic enhanced morphological features of breast MR1 have certain reference value for the diagnosis of TNBC and HER-2 overexpression breast cancer.
