【摘要】：目的:探討發(fā)泡膠個(gè)體化頭枕在頭頸部腫瘤調(diào)強(qiáng)放射治療體位固定中的應(yīng)用價(jià)值。方法:選取92例頭頸部腫瘤患者,共進(jìn)行589次錐形束CT(CBCT)掃描,其中44例患者采用發(fā)泡膠個(gè)體化頭枕體位固定(個(gè)體化組),進(jìn)行了252次CBCT掃描,48例患者采用標(biāo)準(zhǔn)化頭枕體位固定(標(biāo)準(zhǔn)組),進(jìn)行了337次CBCT掃描。分析擺位誤差,對(duì)誤差結(jié)果進(jìn)行分析并記錄成小結(jié)供以后擺位時(shí)參考,記錄兩組患者的擺位誤差結(jié)果并進(jìn)行統(tǒng)計(jì)學(xué)分析。結(jié)果:個(gè)體化組在X、Y和Z軸的線性誤差分別為(0.85±0.68)mm、(1.17±0.87)mm和(1.00±0.81)mm,旋轉(zhuǎn)誤差分別為(0.52±0.45)°、(0.72±0.49)°和(0.57±0.47)°;標(biāo)準(zhǔn)組在X、Y和Z軸的線性誤差分別為(1.15±0.90)mm、(1.48±1.20)mm和(1.08±0.81)mm,旋轉(zhuǎn)誤差分別為(0.68±0.52)°、(0.77±0.55)°和(0.60±0.44)°;個(gè)體化組在X、Y軸的線性誤差、X軸的旋轉(zhuǎn)誤差小于標(biāo)準(zhǔn)組,差異有統(tǒng)計(jì)學(xué)意義(P值分別為0.000、0.001、0.000)。小結(jié)前標(biāo)準(zhǔn)組Y軸的線性誤差大于小結(jié)后(P=0.001),個(gè)體化組小結(jié)前與小結(jié)后的線性誤差和旋轉(zhuǎn)誤差差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:相對(duì)于標(biāo)準(zhǔn)化頭枕,個(gè)體化頭枕可以減少X、Y軸的線性誤差及X軸的旋轉(zhuǎn)誤差,個(gè)體化頭枕體位固定穩(wěn)定,擺位可重復(fù)性高,可在臨床中推廣應(yīng)用。
[Abstract]:Objective: to investigate the application value of styrofoam individualized head pillow in position fixation of head and neck tumor. Methods: 92 patients with head and neck tumor were examined with CT-CBCTs for 589 times. Among them, 44 cases were treated with styrofoam individualized head position fixation (individualized group), and 48 cases with standardized head position fixation were performed CBCT scan for 252times (standard group, 337 times CBCT scan). The error results are analyzed and recorded as a summary for future reference. Results: the linear errors of XY and Z axis in the individualized group were 1.17 鹵0.87 mm and 1.00 鹵0.81 mm respectively, and the rotation errors were 0.52 鹵0.45 擄0.72 鹵0.49 擄and 0.57 鹵0.47 擄, respectively, and the linear errors in the standard group were 0.72 鹵0.49 擄and 0.57 鹵0.47 擄, respectively. The rotation errors were 1.48 鹵1.20mm and 1.08 鹵0.81mm. the rotation errors were 0.68 鹵0.52 擄, 0.77 鹵0.55 擄and 0.60 鹵0.44 擄, respectively. The difference was statistically significant (P = 0.000 ~ 0.001 ~ 0.000 ~ 0.000). The linear error of Y axis in the standard group was higher than that in the back of the nodule, but there was no significant difference in the linear error and the rotation error between the two groups. Conclusion: compared with the standard headrest, the linear error and rotation error of the standard group are not significantly different from those of the standard headrest, and there is no significant difference in the linear error and rotation error between the pre-nodule and the post-nodule in the individualized group. The individualized head pillow can reduce the linear error and the rotation error of the X axis. The position of the individual head pillow is stable and the pendulum position is reproducible. It can be popularized and applied in clinic.
