【摘要】：目的探討腫瘤與正常腦組織表觀擴(kuò)散系數(shù)(ADC)比值診斷兒童后顱窩腫瘤的價(jià)值。方法回顧性分析60例經(jīng)手術(shù)病理證實(shí)的兒童后顱窩腫瘤,其中毛細(xì)胞型星形細(xì)胞瘤(PA)30例,髓母細(xì)胞瘤(MB)18例,室管膜瘤12例。術(shù)前均接受MR檢查,包括DWI。測(cè)定腫瘤實(shí)性部分、正常腦組織ADC值,計(jì)算腫瘤與正常腦組織ADC比值。利用ROC曲線確定ADC比值對(duì)腫瘤鑒別診斷的最佳閾值。結(jié)果腫瘤與正常腦組織ADC比值PA為2.33±0.39,MB為0.96±0.19,室管膜瘤為1.52±0.26,差異有統(tǒng)計(jì)學(xué)意義(P均0.05)。ROC曲線分析顯示區(qū)分室管膜瘤與PA的腫瘤與正常腦組織ADC比值最佳閾值為1.87,其敏感度為91.7%,特異度為86.7%;區(qū)分MB與室管膜瘤的腫瘤與正常腦組織ADC比值最佳閾值為1.19,其敏感度為88.9%,特異度為91.7%。結(jié)論腫瘤與正常腦組織ADC比值對(duì)常見兒童后顱窩腫瘤具有較高的鑒別診斷價(jià)值。
[Abstract]:Objective to evaluate the value of apparent diffusion coefficient (ADCC) ratio of tumor to normal brain in the diagnosis of posterior fossa tumor in children. Methods 60 cases of posterior cranial fossa tumor proved by operation and pathology were retrospectively analyzed, including 30 cases of hair cell astrocytoma. 18 cases of medulloblastoma and 12 cases of ependymoma were examined by Mr before operation, including DWI. The ADC of normal brain tissue was measured. The ADC ratio of tumor to normal brain tissue was calculated. The best threshold value of ADC ratio for differential diagnosis of tumor was determined by ROC curve. Results the ADC ratio of tumor to normal brain tissue was 2.33 鹵0.39 MB = 0.96 鹵0.19, and that of ependymoma was 1.52 鹵0.26, the difference was statistically significant (P. The optimum threshold of ADC ratio between PA and ependymoma is 1.87, the sensitivity is 91.7, the specificity is 86.7, and the optimal threshold of ADC ratio between MB and ependymoma is normal brain tissue. 1.19, the sensitivity was 88.9 and the specificity was 91.7. Conclusion the ratio of tumor to normal brain tissue ADC has a higher value in differential diagnosis of common children's posterior cranial fossa tumors.
