【摘要】：目的探討CEUS對(duì)甲狀腺TI-RADS 3、4級(jí)結(jié)節(jié)的診斷價(jià)值。方法回顧性分析95例常規(guī)超聲診斷為T(mén)IRADS 3、4級(jí)的甲狀腺結(jié)節(jié)(共95個(gè)結(jié)節(jié),均經(jīng)手術(shù)病理證實(shí))的CEUS表現(xiàn),分析CEUS對(duì)甲狀腺TI-RADS 3、4級(jí)結(jié)節(jié)的校正及鑒別診斷價(jià)值。結(jié)果常規(guī)超聲TI-RADS分級(jí)評(píng)估甲狀腺結(jié)節(jié)良惡性與手術(shù)病理級(jí)型比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(χ~2=3.56,P=0.06)。對(duì)于直徑1cm的甲狀腺TI-RADS 3、4級(jí)結(jié)節(jié),與常規(guī)超聲TI-RADS分級(jí)相比,CEUS評(píng)分及CEUS校正后TI-RADS分級(jí)的診斷準(zhǔn)確率差異均有統(tǒng)計(jì)學(xué)意義(P=0.03、0.01);對(duì)于直徑≤1cm的微小乳頭狀癌,差異均無(wú)統(tǒng)計(jì)學(xué)意義(P=0.25、1.00)。ROC曲線分析顯示,對(duì)于甲狀腺TI-RADS 3、4級(jí)結(jié)節(jié),常規(guī)超聲TI-RADS分級(jí)、CEUS評(píng)分、CEUS校正后TI-RADS分級(jí)的ROC曲線下面積分別為0.64、0.75、0.81時(shí),截?cái)嘀捣謩e為T(mén)I-RADS 4a級(jí)、1分、TI-RADS 4a級(jí),評(píng)估結(jié)節(jié)良、惡性的敏感度、特異度分別為45.3%、80.0%,69.3%、65.0%,82.7%、60.0%。CEUS評(píng)分、CEUS校正后TI-RADS分級(jí)與常規(guī)超聲TI-RADS分級(jí)的曲線下面積的差異有統(tǒng)計(jì)學(xué)意義(P均0.05),CEUS評(píng)分與CEUS校正后TI-RADS分級(jí)的曲線下面積差異無(wú)統(tǒng)計(jì)學(xué)意義(P=0.23)。結(jié)論 CEUS對(duì)甲狀腺TIRADS 3、4級(jí)結(jié)節(jié)具有校正及提高鑒別診斷的價(jià)值。
[Abstract]:Objective to evaluate the value of CEUS in the diagnosis of thyroid TI-RADS 3 grade 4 nodule. Methods the CEUS findings of 95 cases of thyroid nodule with grade 3 of TIRADS 3 or grade 4 diagnosed by conventional ultrasound were retrospectively analyzed. The value of CEUS in the correction and differential diagnosis of thyroid TI-RADS 3 or 4 grade nodule was analyzed. Results there was no significant difference between benign and malignant thyroid nodules by conventional ultrasound TI-RADS grading (蠂 2 / 3.56) and thyroid TI-RADS grade 3 / 4 nodule with diameter of 1 cm (蠂 2 / 3.56). Compared with the conventional TI-RADS grade, the diagnostic accuracy of CEUs and TI-RADS after CEUS correction were significantly higher than that of conventional ultrasound, and there was no significant difference in the diagnosis accuracy of P0. 03 or 0. 01% for small papillary carcinoma with diameter 鈮，
