【摘要】：目的探討實時剪切波彈性成像(SWE)技術(shù)在原發(fā)性肝細胞癌(HCC)手術(shù)范圍評估中的臨床應用價值。方法回顧性分析45例經(jīng)術(shù)后病理檢查證實為HCC患者的SWE聲像圖資料。對比HCC病灶,距病灶1、2、3cm肝組織及正常肝組織的彈性模量值,并進行統(tǒng)計學分析。結(jié)果 HCC病灶,距病灶1、2、3cm肝組織及正常肝組織彈性模量的平均值分別為(28.19±6.14)kPa、(17.14±5.34)kPa、(13.44±3.76)kPa、(10.98±3.44)kPa及(10.03±3.69)kPa。HCC病灶,距病灶1、2cm肝組織與距病灶3cm肝組織及正常肝組織間差異均有統(tǒng)計學意義(P均0.05);距病灶1cm肝組織與距病灶2cm肝組織間差異有統(tǒng)計學意義(P0.05);距病灶3cm肝組織與正常組織間差異無統(tǒng)計學意義(P0.05)。2名醫(yī)師對HCC病灶,距病灶1、3cm肝組織及正常肝組織所測彈性模量值差異均無統(tǒng)計學意義(P均0.05),僅對距病灶2cm肝組織彈性模量值差異有統(tǒng)計學意義(P=0.02)。結(jié)論采用SWE技術(shù)測定HCC病灶及其周邊不同范圍肝組織的彈性模量值可為術(shù)前評估手術(shù)切緣范圍提供參考信息。
[Abstract]:Objective to evaluate the clinical value of real time shear wave elastography (SWE) in evaluating the operative range of primary hepatocellular carcinoma (HCC). Methods the sonographic data of 45 patients with HCC proved by pathological examination after operation were retrospectively analyzed and compared with the focus of HCC. Results the mean values of elastic modulus of liver tissue and normal liver tissue in HCC were 28.19 鹵6.14 ~ 6.14 鹵5.34 鹵5.34 KPA 10.98 鹵3.44 KPA and 10.03 鹵3.69 KPA, respectively, and were 13.44 鹵3.44 鹵3.44 鹵3.44 KPA, 10.03 鹵3.69 KPA, respectively, and the mean values of elastic modulus in normal liver tissue and normal liver tissue were 13.44 鹵3.76kPa10.98 鹵3.44kPa and 10.03 鹵3.69kPa.HCC, respectively, with a mean value of 17.14 鹵5.34 KPA, 13.44 鹵3.76kPa10.98 鹵3.44kPa, and 10.03 鹵3.69kPa.HCC, respectively. There were significant differences between liver tissue (1cm to focus), liver tissue (3cm from lesion) and normal liver tissue (P 0.05), between liver tissue (1cm from focus) and liver tissue (2cm from lesion), between liver tissue (3cm from lesion) and normal liver tissue (P < 0.05). There was no statistically significant difference in tissue between two groups (P0.05. 2) for HCC lesions. There was no significant difference in elastic modulus between 1 ~ 3 cm liver tissue and normal liver tissue (P < 0.05), but there was significant difference in elastic modulus value between liver tissue of 2 cm away from focus. Conclusion SWE technique is used to measure HCC lesion and its periphery. The elastic modulus of liver tissue in different range of side can provide reference information for preoperative evaluation of the margin of operation.
