【摘要】：小兒外感后久咳是臨床常見病，宋鐵玎教授從事兒科呼吸系統(tǒng)疾病的臨床工作30余年，根據(jù)多年的臨床觀察和實踐經驗，總結出小兒外感后久咳多是風邪夾寒雜熱夾燥傷肺，肺氣宣降失常，陽氣被郁于內，或外寒入里化熱，又現(xiàn)在小兒因肉食和膏粱厚味之品食之太多較以往體質為熱，生理上脾常不足，飲食過多或不節(jié)，極易傷脾，飲食內停積而化熱，熱邪傷陰耗氣，上中焦肺脾氣陰兩傷，故久咳不止。其他因素還有情志內傷，被動吸煙，環(huán)境污染等。導師根據(jù)肺脾在生理和病理上的子母關系，，提出治療外感后久咳應從肺脾兩臟入手，外感初期，宣發(fā)陽氣，宣肺疏風，給邪以出路，日久則肺脾氣陰皆傷，治當宣肺益氣，健脾助運。西醫(yī)在治療外感后久咳上缺乏特異性手段，切療效不高，中醫(yī)在治療此病上有一定的優(yōu)勢。本文通過對宋鐵玎教授治療小兒外感后久咳的指導思想和臨床經驗介紹，從病因病機、辯證論治等方面豐富了外感后久咳的學術理論和治療方法，對提高臨床醫(yī)生對于小兒外感后久咳的治療療效起到良好的指導作用。
[Abstract]:Chronic cough is a common clinical disease in children. Professor Song Tiedin has been engaged in the clinical work of pediatric respiratory diseases for more than 30 years. Based on many years of clinical observation and practical experience, it is concluded that the most common causes of prolonged cough in children are wind pathogen, cold and miscellaneous heat, dryness and injury to the lung. The lung qi is declared to be abnormal, the Yang qi is depressed in the inside, or the outside cold into the inside heat, and now children because of meat and paste sorghum thick taste of food too much than the previous physique is hot, physiological spleen is often insufficient, eating too much or not, very easy to hurt the spleen, Dietetic accumulation and heat, heat caused by yin gas consumption, on the middle Jiao lung qi Yin two injuries, so a long cough. Other factors are emotional internal injury, passive smoking, environmental pollution and so on. According to the physiological and pathological relationship between the mother and mother of the lung and spleen, the tutor proposed that the chronic cough after treatment of external sensation should start with the lung and spleen two viscera. In the initial period of the exosensory, it is necessary to propagate Yang Qi, clear the lung and soothe the wind, and give the evil spirit a way out. For a long time, both the lung and the spleen are injured, and the lung and qi are treated. Invigorate the spleen and assist in the transportation. Western medicine in the treatment of long cough after the lack of specific means, cutting effect is not high, traditional Chinese medicine in the treatment of this disease has a certain advantage. Based on the introduction of the guiding ideology and clinical experience of Professor Song Tiedin in the treatment of infantile cough, this article enriches the academic theory and treatment methods of post-external cough from the aspects of etiology, pathogenesis, dialectical treatment, and so on. It plays a good guiding role in improving the therapeutic effect of long cough after exosensory in children.
