【摘要】：目的:探討小兒股骨頭缺血性壞死股骨近端旋轉(zhuǎn)內(nèi)翻截骨術(shù)對患兒預(yù)后的分析。方法:2011年1月至2015年3月期間在我院骨科中心接受小兒股骨頭缺血性壞死股骨近端旋轉(zhuǎn)內(nèi)翻截骨術(shù)患兒15例(17髖),其中男9例,女6例;單側(cè)13例,雙側(cè)2例;年齡6~9歲,平均(7.23±1.1)歲。病因:股骨頭缺血性壞死。術(shù)后隨訪運(yùn)用Mckay髖關(guān)節(jié)評分進(jìn)行綜合評分、PACS系統(tǒng)軟件測量骨盆正位片骺高比、髖臼覆蓋率、Sharp角、CE角、Shenton線、股骨頭形態(tài)。結(jié)果:15例患兒均完成隨訪,隨訪時(shí)間10~48個(gè)月,平均(21.2±11.7)月。術(shù)后按照Mckay髖關(guān)節(jié)評分,患兒術(shù)后優(yōu)良率達(dá)80%。根據(jù)Willet骨盆測量法,患兒相關(guān)指標(biāo)均較術(shù)前明顯好轉(zhuǎn)。結(jié)論:對于CatteralⅢ、Ⅳ型患兒股骨旋轉(zhuǎn)內(nèi)翻截骨術(shù)對患兒髖關(guān)節(jié)活動、股骨頭重塑均有較好的幫助。
[Abstract]:Objective: to investigate the prognosis of children with avascular necrosis of femoral head. Methods: from January 2011 to March 2015, 15 children (17 hips) with proximal femoral head avascular necrosis underwent rotatory varus osteotomy in our orthopedic center, including 9 males and 6 females, 13 unilateral and 2 bilateral, aged 69 years. The mean age was (7.23 鹵1.1) years. Cause: avascular necrosis of femoral head. The Mckay hip joint score was used to evaluate the epiphyseal height ratio, the acetabular coverage rate, the CE angle and the shape of the femoral head. Results all 15 cases were followed up for 10 ~ 48 months (mean 21.2 鹵11.7 months). According to Mckay hip score, the excellent and good rate was 80%. According to Willet pelvic measurement, the related indexes of children were improved significantly compared with those before operation. Conclusion: rotational varus osteotomy of femur is helpful to hip motion and femoral head remodeling in children with Catteral type 鈪，
