【摘要】：背景毛細(xì)血管滲漏綜合征（capillary leak syndrome, CLS）是體外循環(huán)心臟手術(shù)后的一種嚴(yán)重并發(fā)癥，是一種可逆性的毛細(xì)血管滲透性增高，血管中的血漿滲出到組織間隙，導(dǎo)致全身進(jìn)行性水腫、低蛋白血癥、休克，甚至出現(xiàn)多器官功能衰竭，死亡率較高，好發(fā)于需較長(zhǎng)時(shí)間體外循環(huán)的心臟手術(shù)后，且在新生兒和嬰幼兒的發(fā)病率較高。 目的探討低體重嬰幼兒先天性心臟病體外循環(huán)術(shù)后發(fā)生毛細(xì)血管滲漏綜合征的相關(guān)危險(xiǎn)因素。 方法回顧性分析沈陽軍區(qū)總醫(yī)院2008年10月～2010年2月的204例體重≤13kg的先天性心臟病體外循環(huán)術(shù)后患兒的臨床資料，對(duì)可能導(dǎo)致CLS發(fā)生的因素進(jìn)行單因素分析以及多元Logistic回歸分析，確定與CLS發(fā)病相關(guān)的獨(dú)立危險(xiǎn)因素。 結(jié)果本組資料體外循環(huán)術(shù)后出現(xiàn)CLS26例，發(fā)生率為12.75%，單因素分析提示CLS組的年齡、體重、自體血容量/預(yù)充量（autoblood volume/primingvolume,A/P）、術(shù)中最低溫度明顯低于非CLS組，體外循環(huán)時(shí)間、主動(dòng)脈阻斷時(shí)間、先天性心臟病手術(shù)危險(xiǎn)分級(jí)（risk adjustment in congenital heartsurgery, RACHS）明顯高于非CLS組。經(jīng)多元Logistic回歸分析顯示A/P值是CLS發(fā)生的最主要影響因素。 結(jié)論A/P值是CLS發(fā)生的主要獨(dú)立危險(xiǎn)因素，體重在CLS發(fā)生中具有一定意義。
