【摘要】：目的評(píng)價(jià)基礎(chǔ)血清黃體生成素(LH)對(duì)女性中樞性性早熟(CPP)的診斷價(jià)值。方法以279例女性性早熟患兒為研究對(duì)象,其中CPP 175例、單純?nèi)榉吭绨l(fā)育(PT)104例,均行體格生長評(píng)價(jià)、骨齡測定以及基礎(chǔ)LH、卵泡刺激素(FSH)檢測和性激素激發(fā)試驗(yàn)等。采用受試者工作曲線(ROC)分析基礎(chǔ)LH、FSH及其峰值對(duì)診斷CPP的意義,并分析基礎(chǔ)LH與LH峰值的相關(guān)性。結(jié)果 CPP組的骨齡,基礎(chǔ)LH、FSH,以及LH和FSH峰值,LH/FSH峰值之比均較PT組升高(P0.01)。以基礎(chǔ)LH和LH峰值診斷效果較好�；A(chǔ)LH對(duì)骨齡7.0~9.0歲,9.0~11.0歲,11.0歲3個(gè)CPP亞組的診斷價(jià)值以11.0歲組的曲線下面積(AUC)最大�；A(chǔ)LH在0.45 IU/L時(shí)Youden指數(shù)最大,靈敏度為0.667、特異度為0.8;LH峰值在9.935 IU/L時(shí)Youden指數(shù)最大,靈敏度為0.748、特異度為1�；A(chǔ)LH與LH峰值成正相關(guān)(r=0.440、P0.01)。結(jié)論基礎(chǔ)LH可作為診斷CPP的初篩指標(biāo),在不同骨齡階段均有一定的診斷價(jià)值,并可作為療效監(jiān)測指標(biāo)。
[Abstract]:Objective to evaluate the value of basic serum luteinizing hormone (LH) in the diagnosis of central precocious puberty (CPP) in women. Methods 279 cases of female precocious precocious puberty were studied, including 175 cases of CPP and 104 cases of simple breast development (PT). The physical growth evaluation, bone age measurement, basic LH, follicle stimulating hormone (FSH) and sex hormone stimulation test were performed. Using the subjects' work curve (ROC) to analyze the significance of basic LH, FSH and its peak value for the diagnosis of CPP, and to analyze the correlation between the basic LH and the peak value of LH. Results the bone age of the CPP group, the basic LH, FSH, and the LH and FSH peaks are all higher than those of the peak value of the LH/FSH. The diagnostic value of 1 years old, 11 year old and 3 CPP subgroups was the largest in the area under the curve of the 11 year old group (AUC). The Youden index of the base LH at 0.45 IU/L was the largest, the sensitivity was 0.667, the specificity was 0.8; the peak value of LH was 9.935 IU/L, the Youden index was the largest, the sensitivity was 0.748, the specificity was 1. base LH and LH peak was positive correlation (r=0.440, P0.01). It can be used as a screening index for diagnosing CPP. It has certain diagnostic value in different bone age stages and can be used as a monitoring index for curative effect.
