【摘要】：目的:通過對接受人工耳蝸植入術的語前聾患兒的術后聽覺能力及言語能力進行評估,研究分析腦白質異常的雙側極重度感音神經性聾的語前聾患兒在人工耳蝸植入術后的康復效果。方法:對福建省某三甲醫(yī)院自2014年1月至2015年1月之間行人工耳蝸植入術的11例伴腦白質異常(研究組)及18例腦組織正常(對照組)的雙側極重度感音神經聾的語前聾患兒進行回顧性的分析研究。所有患兒術前均行病史調查、聽力學檢查、影像學檢查及常規(guī)檢查。所有患兒手術均由福建省某三甲醫(yī)院耳鼻喉科同一人完成,全部選擇右側患耳植入人工耳蝸,術中與術后均無并發(fā)癥,均于術后1個月開機,所有患兒術后均以當?shù)乜祻椭行难哉Z訓練為主,家庭言語訓練為輔。采用聽覺行為分級標準(CAP)和言語可懂度分級標準(SIR)作為評估標準,在患兒術后6個月及12個月時,對患兒生活環(huán)境中的有直接接觸的家庭成員進行電話隨訪,評估患兒的聽覺能力及言語能力,對研究組及對照組在兩個時間點的指標均數(shù)比較采用t檢驗,p0.05為差異有統(tǒng)計學意義。結果:研究組患兒術后6個月時CAP平均分級為2.41±0.47級,CAP分級均在2-3級之間,而對照組患兒術后6個月時CAP平均分級為3.28±0.45級,CAP分級則大部分位于3-4級之間。研究組患兒術后6個月SIR平均分級為1.27±0.44級,僅3例患兒的SIR分級能夠到達2級,其余均為1級,而對照組患兒術后6個月SIR平均分級為1.89±0.31級,僅2例為1級,其余均為2級。研究組患兒術后12個月CAP平均分級為4.00±0.43級,僅1例分級為3級,1例為5級,其余均為4級,對照組患兒術后12個月CAP平均分級為4.22±0.63級,僅2例為3級,有6例可達5級,其余均為4級。研究組患兒術后12個月SIR平均分級為2.36±0.48級,均在2-3級,對照組患兒術后SIR平均分級為2.44±0.50級,均在2-3級。術后6個月CAP平均分級兩組有統(tǒng)計學差異(t=4.983,P0.05),術后6個月SIR平均分級兩組有統(tǒng)計學差異(t=4.371,P0.05);術后12個月CAP平均分級兩組無統(tǒng)計學差異(t=1.033,P0.05),術后12個月SIR平均分級兩組無統(tǒng)計學差異(t=0.434,P0.05)。結論:在術前MRI檢查里提示腦白質異常的雙側極重度感音神經性聾且無明顯的神經系統(tǒng)癥狀及精神異常等情況的語前聾兒童,其人工耳蝸植入術后康復效果與腦白質正常的患兒無明顯差異。
[Abstract]:Objective: to evaluate the hearing and speech ability of children with prelingual deafness after cochlear implantation.The effects of cochlear implantation in children with bilateral extremely severe sensorineural deafness with abnormal white matter were analyzed.Methods: from January 2014 to January 2015, 11 patients with white matter abnormalities and 18 patients with normal brain tissue (control group) underwent cochlear implantation from January 2014 to January 2015 in a third Class A Hospital in Fujian Province, with bilateral extremely severe sensorineural deafness.A retrospective study was conducted in children with prelingual deafness.All children were examined with history, audiology, imaging and routine examination before operation.All the children underwent the operation by the same person in the department of otolaryngology of the third Class A Hospital in Fujian Province. All the patients were selected for cochlear implantation in the right ear. There were no complications in the operation and the operation was performed 1 month after operation.All the children were trained mainly by local rehabilitation center and supplemented by family language training.Auditory behavior scale (CAP) and speech intelligibility scale (SIRs) were used as the evaluation criteria. The family members who had direct contact with each other were followed up by telephone at 6 and 12 months after operation.To evaluate the hearing ability and speech ability of the children, there was a significant difference between the study group and the control group by using t test (p0.05) to compare the mean of indexes at two time points.Results: in the study group, the average CAP grade was 2.41 鹵0.47 grade and 2-3 grade at 6 months after operation, while in the control group, the average CAP grade was 3.28 鹵0.45 grade and 3.28 鹵0.45 grade respectively.In the study group, the average SIR grade was 1.27 鹵0.44 at 6 months after operation. The SIR grade of the 3 cases could reach grade 2 and the rest were all grade 1. In the control group, the average SIR grade was 1.89 鹵0.31 at 6 months after operation, only 2 cases were grade 1, and the rest were all grade 2.The rest are of grade 4.The average SIR grade was 2.36 鹵0.48 grade (2-3 grade) in the study group and 2.44 鹵0.50 grade (2.44 鹵0.50) in the control group.鏈悗6涓湀CAP騫沖潎鍒嗙駭涓ょ粍鏈夌粺璁″宸紓(t=4.983,P0.05),鏈悗6涓湀SIR騫沖潎鍒嗙駭涓ょ粍鏈夌粺璁″宸紓(t=4.371,P0.05);鏈悗12涓湀CAP騫沖潎鍒嗙駭涓ょ粍鏃犵粺璁″宸紓(t=1.033,P0.05),鏈悗12涓湀SIR騫沖潎鍒嗙駭涓ょ粍鏃犵粺璁″宸紓(t=0.434,P0.05).Conclusion: preoperative MRI examination suggested that children with bilateral extremely severe sensorineural deafness with abnormal white matter and no obvious nervous system symptoms and mental abnormalities were found to have prelingual deafness.There was no significant difference between the rehabilitation effect of cochlear implant and that of normal white matter.
