【摘要】：目的探討外展支具早期治療髖關(guān)節(jié)發(fā)育不良(developmental dysplasia of the hip,DDH)的療效,并分析影響療效的因素。方法回顧性分析我院2013年10月至2015年6月收治的223例(270髖)髖關(guān)節(jié)發(fā)育不良患兒資料,其中男性53例,女性170例;年齡1~183(62.41±23.82)d,中位年齡58 d;左側(cè)151髖,右側(cè)119髖,單側(cè)142髖,雙側(cè)64髖;超聲Graf分型:Ⅱa型143髖,Ⅱb型75髖,Ⅱc型11髖,D型13髖,Ⅲ型19髖,Ⅳ型9髖,均采用外展支具治療,每隔3~4周超聲監(jiān)測髖關(guān)節(jié)形態(tài)變化,6個月行X線攝片,支具治療時間3~20周,隨訪時間12~24個月,平均16.4個月。結(jié)果 206例(250髖)經(jīng)外展支具治療后恢復(fù)正常形態(tài),且無一例發(fā)生股骨頭壞死;17例(20髖)治療失敗,其中17髖改行石膏固定術(shù)治療,3髖行Salter骨盆截骨手術(shù)治療。結(jié)論外展支具是早期治療髖關(guān)節(jié)發(fā)育不良安全、有效的方法之一,對GrafⅡ型髖關(guān)節(jié)治愈率高,對Ⅲ型及Ⅳ型治愈率低,但可嘗試作為初始治療的方法;超聲檢查對于早期診治髖關(guān)節(jié)發(fā)育不良具有重要意義。
