【摘要】：目的將右美托咪定用于兒童牙科日間全麻手術(shù)的維持,探討其對蘇醒期躁動發(fā)生的影響。方法將擬全麻下行口腔治療的80例兒童牙科患兒隨機(jī)分為A組和B組,每組40例。A組在全麻維持期靜脈注射1μg·kg-1負(fù)荷量右美托咪定(生理鹽水稀釋至10 m L),然后以0.1~0.4 m L·(kg·h)-1的速度靜脈注射至手術(shù)結(jié)束前45 min,B組以同樣方式注射生理鹽水。記錄2組患兒的年齡,性別,體重,美國麻醉醫(yī)師協(xié)會病情分級(ASAPS),不同時間點(diǎn)的心率(HR)、平均動脈壓(MAP)、氧飽和度(Sp O2),舒芬太尼用量,手術(shù)持續(xù)時間,手術(shù)結(jié)束到拔管的時間,拔管到清醒的時間、清醒后行為評分(FPAS),達(dá)離院標(biāo)準(zhǔn)的時間等指標(biāo),并進(jìn)行統(tǒng)計分析。結(jié)果 2組手術(shù)開始、手術(shù)10 min、30 min、1 h、2 h及拔管后的HR、MAP均有統(tǒng)計學(xué)差異,A組低于B組(P0.05);舒芬太尼用量及蘇醒期躁動率也有統(tǒng)計學(xué)差異,A組低于B組(P0.05);2組拔管到清醒的時間及達(dá)離院標(biāo)準(zhǔn)的時間為A組長于B組(P0.05)。其他指標(biāo)2組均無統(tǒng)計學(xué)差異。結(jié)論右美托咪定用于兒童牙科日間全麻手術(shù),能降低蘇醒期躁動發(fā)生率,術(shù)中生命體征更平穩(wěn)。
[Abstract]:Objective to use dexmetidine in maintenance of children with day general anesthesia. Methods 80 cases of children dental children undergoing oral treatment under general anesthesia were randomly divided into group A and group B. Each group (40 cases) received intravenous injection of 1 渭 g 路kg-1 load of dexmetidine (diluted to 10 mL with normal saline) during the maintenance period of general anesthesia. Then, 0.4 mL 路kg 路h ~ (-1) of 0.1 渭 m 路L ~ (-1) 路h ~ (-1) was injected intravenously into group B 45 min before the end of operation. The age and sex of the children in the two groups were recorded. Body weight, ASAPS, mean arterial pressure MAPP, oxygen saturation, sufentanil dosage. The duration of operation, the time from the end of operation to extubation, the time from extubation to waking, the behavior score after waking up to FPASA, the time to reach the standard of leaving hospital, etc. Results there were significant differences in HRMAP between the two groups at the beginning of operation, 10 min to 30 min, 1 hour and 2 h after extubation. Group A was lower than group B (P 0.05); There were statistical differences in the dosage of sufentanil and the restlessness rate during the recovery period. The level of sufentanil in group A was lower than that in group B (P 0.05). The time between extubation and sobriety and the time to reach the hospital standard in group A was longer than that in group B (P 0.05). There was no significant difference in other indexes between the two groups. Conclusion dexmetomidine is used in general anesthesia in children's dentistry. It can reduce the incidence of restlessness during recovery period, and the vital signs are more stable during operation.
