【摘要】：目的探討門(mén)診急性腹瀉患者的病原結(jié)果及大便培養(yǎng)確診細(xì)菌性痢疾病例的臨床特點(diǎn)。方法回顧性分析2012年4月至2015年10月從首都醫(yī)科大學(xué)宣武醫(yī)院腸道門(mén)診急性腹瀉患者中,隨機(jī)抽樣的1280例患者大便培養(yǎng)結(jié)果及臨床資料。結(jié)果 1280例大便標(biāo)本共檢出5種主要腸道致病菌180株,志賀菌屬培養(yǎng)陽(yáng)性率為3.8%(49/1280),其中宋內(nèi)菌36例(占73.5%),福氏菌13例(占26.5%)。49例細(xì)菌性痢疾大便培養(yǎng)陽(yáng)性患者中,大便常規(guī)達(dá)到細(xì)菌性痢疾診斷標(biāo)準(zhǔn)者不到20%(占18.4%);而臨床表現(xiàn)有高熱者(T≥38.5℃)約占1/3(28.6%),明顯高于培養(yǎng)出非志賀菌屬組或未培養(yǎng)出細(xì)菌組(P0.05)。大便常規(guī)提示細(xì)菌性痢疾的132例患者中,大便培養(yǎng)志賀菌屬陽(yáng)性者不到10%(6.8%)。結(jié)論細(xì)菌性痢疾中宋內(nèi)菌占絕對(duì)優(yōu)勢(shì),大便培養(yǎng)確診的細(xì)菌性痢疾患者的臨床特點(diǎn)(除發(fā)熱外)與其他急性腹瀉患者相比并無(wú)特異性。
[Abstract]:Objective to investigate the pathogenic results of outpatients with acute diarrhea and the clinical characteristics of bacterial dysentery diagnosed by stool culture. Methods from April 2012 to October 2015, 1280 patients with acute diarrhea were randomly selected from Xuanwu Hospital of Capital Medical University. The results of stool culture and clinical data were analyzed retrospectively. Results A total of 180 strains of 5 major intestinal pathogenic bacteria were detected in 1280 stool specimens. The positive rate of Shigella culture was 3.8% (49 / 1280), including 36 cases (73.5%) of Soonella and 13 cases (26.5%) of flexneria.
