【摘要】：目的分析某院2013-2015年肺結(jié)核患者住院費用,探討其影響因素,為減輕肺結(jié)核患者經(jīng)濟負擔(dān)提供參考。方法收集該院2013年10月-2015年12月間首要診斷為肺結(jié)核的住院患者的病案首頁共543份,對其進行回顧性分析。結(jié)果該院2013-2015年肺結(jié)核患者住院費用的平均水平為6 420.61元,其中藥品類費用占59.32%,診斷類費用占24.86%。相關(guān)性分析結(jié)果表明,住院天數(shù)與住院費用呈正相關(guān),年齡因素與住院費用不相關(guān)。非參數(shù)檢驗結(jié)果顯示不同痰鏡檢結(jié)果、合并疾病、出院情況以及付費方式的患者,其住院費用不同。多因素分析結(jié)果顯示,在控制其他因素的情況下,不同出院情況的人群住院費用之間的差異無統(tǒng)計學(xué)意義。結(jié)論該院肺結(jié)核患者的住院費用低于同期其他地區(qū)醫(yī)院,藥品類費用所占比例較高,住院天數(shù)、痰鏡檢結(jié)果、合并疾病、付費方式等因素為肺結(jié)核患者住院費用的主要影響因素。建議醫(yī)院對結(jié)核病種實施臨床路徑管理,減少無效住院天數(shù),控制住院費用;同時注重防治合并疾病,做好預(yù)防保健工作。
[Abstract]:Objective to analyze the hospitalization expenses of pulmonary tuberculosis patients in a hospital from 2013 to 2015, and to explore the influencing factors, and to provide reference for alleviating the economic burden of pulmonary tuberculosis patients. Methods from October 2013 to December 2015, 543 medical records of inpatients with pulmonary tuberculosis were collected and analyzed retrospectively. Results the average hospitalization cost of pulmonary tuberculosis patients in the hospital from 2013 to 2015 was 6 420.61 yuan, of which the cost of drugs accounted for 59.32, and the cost of diagnosis was 24.86 yuan. The results of correlation analysis showed that there was a positive correlation between hospitalization days and hospitalization expenses, but no correlation between age factors and hospitalization expenses. The results of nonparametric examination showed that patients with different sputum microscopic examination, complicated diseases, discharge status and payment methods had different hospitalization costs. The results of multivariate analysis showed that there was no significant difference in hospitalization cost among different discharge patients under the control of other factors. Conclusion the hospitalization cost of pulmonary tuberculosis patients in this hospital is lower than that in other hospitals in the same period, the proportion of drug expenses is higher, the days of hospitalization, the results of sputum microscopy, and the complication of diseases. The factors such as payment method are the main factors influencing the hospitalization expenses of pulmonary tuberculosis patients. It is suggested that hospitals should implement clinical path management to tuberculosis species, reduce invalid hospitalization days, control hospitalization expenses, and pay attention to prevention and treatment of combined diseases, and do a good job of prevention and health care.
