【摘要】：目的評價夾脊穴注射糖皮質(zhì)激素對預防50歲以上患者帶狀皰疹后遺神經(jīng)痛的療效及安全性。方法計算機檢索Cochrane Library、Pub Med、EMbase、CNKI、CBM、Wan Fang和VIP(從建庫至2015年10月),納入夾脊穴注射糖皮質(zhì)激素或椎旁神經(jīng)阻滯與空白或安慰劑對照治療急性帶狀皰疹的隨機對照試驗,治療組與對照組可結(jié)合或不結(jié)合基礎治療。由2名研究者按照Cochrane Handbook標準獨立評價文獻質(zhì)量、提取數(shù)據(jù)并交叉核對,使用Rev Man5.3軟件進行Meta分析。結(jié)果共納入4個研究,合計1 374例患者。Meta分析結(jié)果顯示:6個月時PHN的發(fā)生率試驗組低于對照組[RR=0.39,95%CI(0.16,0.94)];除對照組出現(xiàn)1例嚴重不良反應外,其余均未報道嚴重不良反應。結(jié)論基于目前的證據(jù),夾脊穴注射糖皮質(zhì)激素可能預防或降低50歲以上患者PHN的發(fā)生率,且安全較好,但尚缺乏更多高質(zhì)量的研究,目前還不能得到更為可靠的結(jié)論。
[Abstract]:Objective to evaluate the efficacy and safety of injection of glucocorticoid at Jiaji acupoint in the prevention of postherpetic neuralgia in patients over 50 years old. Methods Cochrane Library Pub Medbase Cochrane Fang and VIPs were searched by computer. A randomized controlled trial of corticosteroid or paravertebral nerve block in the treatment of acute herpes zoster with blank or placebo control. The treatment group and the control group could be combined with or without basic treatment. The quality of the literature was evaluated independently by two researchers according to Cochrane Handbook standard, the data were extracted and cross-checked, and the Meta analysis was carried out using Rev Man5.3 software. The results were included in 4 studies. The results of Meta-analysis of 1 374 patients showed that the incidence of PHN in the experimental group was lower than that in the control group at 6 months [RRN 0.39 ~ 95 CI0.160.94], except for one serious adverse reaction in the control group, no severe adverse reactions were reported. Injection of glucocorticoid at Jiaji point may prevent or reduce the incidence of PHN in patients over 50 years old, and it is safe and safe, but there is still a lack of more high quality research, so it is not possible to draw a more reliable conclusion at present.
