【摘要】：目的探討支氣管內(nèi)超聲引導(dǎo)針吸活檢術(shù)(EBUS-TBNA)在肺門、縱膈淋巴結(jié)惡性腫瘤和結(jié)核診斷中的應(yīng)用價(jià)值。方法回顧性分析2013年1月至2016年9月在四川大學(xué)華西醫(yī)院呼吸內(nèi)鏡中心接受EBUSTBNA檢查的553例患者的臨床及病理資料,統(tǒng)計(jì)EBUS-TBNA對(duì)肺門、縱膈淋巴結(jié)腫瘤和結(jié)核的診斷敏感性、特異性和準(zhǔn)確率。結(jié)果 EBUS-TBNA診斷肺門、縱膈淋巴結(jié)惡性腫瘤的敏感性、特異性和準(zhǔn)確率分別為89.2%(263/295)、100.0%(247/247)和94.1%(510/542)。針吸活檢組織查見肉芽腫診斷結(jié)核的敏感性、特異性和準(zhǔn)確率分別為65.0%(76/117)、97.2%(385/396)和89.9%(461/513)。標(biāo)本組織行抗酸染色和TB-PCR的102例中,查見抗酸桿菌或TB-PCR任一項(xiàng)陽性診斷結(jié)核的敏感性、特異性和診斷準(zhǔn)確率為63.7%(58/91)、90.9%(10/11)和66.7%(68/102)。結(jié)論 EBUS-TBNA診斷肺門和縱膈腫瘤具有較高的敏感性和特異性,并可聯(lián)合抗酸染色和TB-PCR聯(lián)合診斷肺門和縱膈淋巴結(jié)結(jié)核。
[Abstract]:Objective to explore the role of EBUS-TBNA-EBUS-TBNAA in the pulmonary hilus. Methods from January 2013 to September 2016, the clinical and pathological data of 553 patients undergoing EBUSTBNA examination in the Center of Respiratory Endoscopy, West China Hospital, Sichuan University, were retrospectively analyzed. The sensitivity, specificity and accuracy of EBUS-TBNA in diagnosis of hilar and mediastinal lymph node tumors and tuberculosis were analyzed. Results the sensitivity of EBUS-TBNA in the diagnosis of hilar and mediastinal lymph node malignant tumors was evaluated. The specificity and the accuracy were 89. 2 / 263 / 295 100.010. 0 and 94. 1% 510 / 542, respectively. The specificity and accuracy of needle aspiration biopsy were 65. 0% 767% 97. 2% 3855% 396 and 89. 9% 461 513%, respectively. There were 102 cases of antacid staining and TB-PCR. The sensitivity, specificity and diagnostic accuracy of either positive acid-fast bacilli or TB-PCR for diagnosis of tuberculosis were 63.7% and 66.7%, respectively. Conclusion EBUS-TBNA has a high sensitivity and specificity in the diagnosis of hilar and mediastinal tumors. It can be combined with acid-fast staining and TB-PCR to diagnose hilar and mediastinal lymph node tuberculosis.
