【摘要】：目的評估瞬時彈性超聲成像(Fibro Scan)在慢性乙型肝炎(CHB)患者肝臟纖維化診斷中的臨床應用價值。方法選取進行肝臟穿刺活檢的CHB患者185例,收集血常規(guī)、肝功能等血清學指標。同時患者應用Fibro Scan進行肝臟彈性測量值(LSM)測量,分析LSM與肝纖維化的相關性及其影響因素。結果以肝臟穿刺病理結果為金標準,Spearman等級相關性分析提示LSM與肝纖維化呈顯著相關性(P0.05)。單因素方差分析提示天門冬氨酸氨基轉移酶、堿性磷酸酶、血小板、凝血酶原時間百分活動度與LSM值呈相關性。受試者工作特性曲線(ROC)分析,S2~S4期診斷界值依次為6.0、9.75、14.2 k Pa,各階段曲線下面積均0.95。雙變量分析提示LSM值與升高的轉氨酶呈顯著相關性(P0.05)。ROC分析轉氨酶升高組S1~S2期界值為6.25 k Pa,S3~S4期界值為15.3 k Pa。結論 Fibro Scan診斷CHB患者肝臟纖維化有較高的臨床應用價值,但是受轉氨酶影響大。
[Abstract]:Objective to evaluate the clinical value of fibro Scann in the diagnosis of hepatic fibrosis in patients with chronic hepatitis B. methods 185 CHB patients with liver biopsy were selected and blood routine was collected. At the same time, Fibro Scan was used to measure the liver elasticity. The correlation between LSM and hepatic fibrosis and its influencing factors were analyzed. Results according to the pathological results of liver puncture as the golden standard, the correlation between LSM and liver fibrosis was significant (P 0.05), and the single factor variance analysis suggested that aspartate aminotransferase. Alkaline phosphatase, platelets, There was a correlation between prothrombin time activity and LSM value. The diagnostic threshold value of S2S4 phase was 6.0 ~ 9.75 ~ 14.2 kPa. the area under each phase curve was 0.95.Bivariate analysis showed that LSM value was associated with elevated aminotransferase. There was a significant correlation between P0.05 and ROC in the analysis of transaminase in patients with elevated aminotransferase. The threshold of S _ 1 / S _ 2 was 6.25 k / s _ 3s _ 3 / S _ 4, which was 15.3kPa.Conclusion Fibro Scan has a high clinical value in diagnosing hepatic fibrosis in patients with CHB. But it is heavily affected by transaminase.
