機(jī)械通氣致兔肺損傷后TNF-α、IL-8時(shí)序性表達(dá)規(guī)律的研究

【摘要】:研究目的及背景. 腫瘤壞死因子-α(TNF-α)和白介素-8(IL-8)在創(chuàng)傷性急性肺損傷、創(chuàng)傷性休克致急性肺損傷均有表達(dá),但迄今為止,過(guò)度機(jī)械通氣致肺損傷后TNF-α和IL-8的表達(dá)時(shí)序性規(guī)律尚未見(jiàn)報(bào)道。本研究主要是對(duì)兔進(jìn)行過(guò)度機(jī)械通氣后TNF-α和IL-8的表達(dá)規(guī)律進(jìn)行研究,并且對(duì)機(jī)械通氣性肺損傷的分子機(jī)制進(jìn)行了進(jìn)一步的了解,也為機(jī)械通氣致肺損傷的分子病理診斷、法醫(yī)學(xué)鑒定提供理論依據(jù)。 方法: 將健康新西蘭兔隨機(jī)分為空白組、對(duì)照組及實(shí)驗(yàn)組。實(shí)驗(yàn)組按照損傷后處死時(shí)間分為0小時(shí)、0.5小時(shí)、1小時(shí)、1.5小時(shí)、3小時(shí)、6小時(shí)、12小時(shí)、18小時(shí)和24小時(shí)9個(gè)亞組。應(yīng)用組織芯片、免疫組化及免疫蛋白印跡方法對(duì)兔肺組織中的TNF-α、IL-8的表達(dá)進(jìn)行研究。 結(jié)果: (1)空白組及對(duì)照組各時(shí)間點(diǎn)肺組織內(nèi)均有低水平的TNF-α、IL-8表達(dá),它們之間的差異無(wú)統(tǒng)計(jì)學(xué)意義。(2)機(jī)械通氣后TNF-α、IL-8表達(dá)明顯增加,較對(duì)照組比較有統(tǒng)計(jì)學(xué)意義。(3)機(jī)械性通氣致肺損傷后:TNF-α 0.5小時(shí)上升,1.5小時(shí)達(dá)高峰,,2小時(shí)下降,6小時(shí)后接近對(duì)照組水平。IL-8 1.5小時(shí)開(kāi)始升高,6小時(shí)仍繼續(xù)上升,12小時(shí)開(kāi)始下降,24小時(shí)達(dá)對(duì)照組。 結(jié)論: 機(jī)械通氣致肺損傷后可誘導(dǎo)TNF-α、IL-8在肺組織內(nèi)表達(dá)增加,TNF-α、IL-8的表達(dá)改變的時(shí)序性規(guī)律可望作為一種客觀指標(biāo)用于法醫(yī)學(xué)機(jī)械性肺損傷的早期損傷經(jīng)歷時(shí)間的推斷。
[Abstract]:Objective and background. Tumor necrosis factor-偽 (TNF- 偽) and IL-8 (IL-8) have been expressed in both traumatic acute lung injury and traumatic shock induced acute lung injury, but up to now, the timing of TNF- 偽 and IL-8 expression after lung injury induced by hypermechanical ventilation has not been reported. The purpose of this study was to study the expression of TNF- 偽 and IL-8 after hypermechanical ventilation in rabbits, and to further understand the molecular mechanism of mechanical ventilation induced lung injury, and to provide theoretical basis for molecular pathological diagnosis and forensic identification of lung injury induced by mechanical ventilation. Methods: healthy New Zealand rabbits were randomly divided into blank group, control group and experimental group. According to the time of death after injury, the experimental group was divided into 9 subgroup: 0 hours, 0.5 hours, 1 hour, 1.5 hours, 3 hours, 6 hours, 12 hours, 18 hours and 24 hours. The expression of TNF- 偽 and IL-8 in rabbit lung tissue was studied by tissue microarray, immunohistochemistry and immunoblotting. Results: (1) there were low levels of TNF- 偽 and IL-8 expression in lung tissue of blank group and control group at each time point, and there was no significant difference between them. (2) the expression of TNF- 偽 and IL-8 increased significantly after mechanical ventilation, which was significantly higher than that of control group. (3) after lung injury induced by mechanical ventilation, TNF- 偽 increased 0. 5 hours. Il-8 reached the peak at 1.5 hours, decreased at 2 hours, and approached the level of the control group after 6 hours. Il-8 began to rise at 1.5 hours, continued to rise at 6 hours, began to decrease at 12 hours, and reached the control group at 24 hours. Conclusion: after lung injury induced by mechanical ventilation, the expression of TNF- 偽 and IL-8 in lung tissue is increased. The timing of the expression of TNF- 偽 and IL-8 is expected to be used as an objective index to infer the early injury time of mechanical lung injury in forensic medicine.
