【摘要】：【目的】研究側(cè)顱底手術(shù)徑路之一—顳盂入路的相關(guān)應(yīng)用解剖，為此手術(shù)入路提供翔實(shí)的解剖學(xué)理論基礎(chǔ)和實(shí)踐依據(jù)。 【方法】通過(guò)對(duì)5具成人濕性尸頭模擬顳盂入路聯(lián)合乳突或乳突—枕下—頸入路進(jìn)行解剖，明確手術(shù)徑路中穿經(jīng)結(jié)構(gòu)以及重要血管神經(jīng)的相互毗鄰關(guān)系；通過(guò)對(duì)53具成人顱骨觀察測(cè)量側(cè)顱底區(qū)骨性結(jié)構(gòu)的臨床應(yīng)用解剖，為手術(shù)中準(zhǔn)確定位側(cè)顱底相關(guān)重要解剖結(jié)構(gòu)，保全顱神經(jīng)和重要血管提供較翔實(shí)的解剖學(xué)依據(jù)。 【結(jié)果】①顳盂相關(guān)解剖：以矢狀面為基準(zhǔn)，顳盂前后徑為顴突關(guān)節(jié)結(jié)節(jié)最下點(diǎn)至鼓乳裂最下點(diǎn)間最短距離，左側(cè)為30.3±2.3mm、右側(cè)為30.4±2.4mm，顳盂左右徑為關(guān)節(jié)后突最下點(diǎn)至頸靜脈窩最短距離，左側(cè)為21.0±2.4mm、右側(cè)為20.1±2.4mm，下頜關(guān)節(jié)窩的前后徑為顴突關(guān)節(jié)結(jié)節(jié)最下點(diǎn)至關(guān)節(jié)后突最下點(diǎn)間最短距離，左側(cè)為18.0±2.0mm、右側(cè)為17.8±1.9mm。②顳盂毗鄰相關(guān)解剖：顳盂外側(cè)起始標(biāo)志關(guān)節(jié)后突最下點(diǎn)、鼓乳裂最下點(diǎn)這兩點(diǎn)與側(cè)顱底重要神經(jīng)血管穿經(jīng)孔隙(莖突根、莖乳孔外緣、頸靜脈球窩外緣、頸動(dòng)脈管口外緣、舌下神經(jīng)管外口外緣、枕骨髁外緣、卵圓孔外緣、棘孔外緣、蝶棘)的最短距離和這兩點(diǎn)與這些孔隙的夾角分別在表1～4中描述。③頸靜脈球窩解剖：測(cè)量分別以冠狀面、矢狀面為基準(zhǔn)的最長(zhǎng)徑線，左側(cè)為14.2±2.1mm、7.3±1.6mm，右側(cè)為15.5±2.0mm、9.7±2.0mm。左右兩側(cè)對(duì)比經(jīng)卡方檢驗(yàn)結(jié)果P＜0.05，有統(tǒng)計(jì)學(xué)意義，，可以認(rèn)為頸靜脈球窩左右兩側(cè)大小不同。④莖乳孔至頸靜脈球窩間最短距離，左
[Abstract]:[objective] to study the applied anatomy of temporopelvic approach, one of the operative approaches of lateral skull base, and to provide a theoretical and practical basis for the surgical approach. [methods] five adult cadaveric cadavers were dissected via the combined mastoid or mastoid-suboccipital cervical approach to determine the structure of the surgical approach and the relationship between the important vessels and nerves. The clinical applied anatomy of bone structure in lateral skull base region was observed and measured in 53 adult skulls, which provided a more accurate anatomic basis for locating the important anatomical structure of lateral skull base and preserving cranial nerves and important blood vessels. [results] 1Temporopelvic related anatomy: according to the sagittal plane, the anterior and posterior diameters of temporal pelvis were the shortest distance between the lowest point of zygomaticoid process joint nodule and the lowest point of tympanomastoid fissure, the left side was 30.3 鹵2.3 mm, the right side was 30.4 鹵2.4 mm. The right and left diameters of temporal pelvis were the shortest distance from the lowest point of posterior process to the fossa of jugular vein, 21.0 鹵2.4 mm on the left and 20.1 鹵2.4 mm on the right. The anteroposterior diameter of the mandibular fossa was the shortest distance between the lowest point of zygomaticoid process joint nodule and the lowest point of posterior articular process. The left side was 18.0 鹵2.0 mm, and the right side was 17.8 鹵1.9mm.2 adjacent anatomy of the temporal glenoid: the lateral temporopelvic origin marks the inferior point of the articular process, the lowest point of the tympanic and mammary fissure, and the important nerve vessels of the lateral skull base through the pore (styloid root). The external margin of stylomastoid foramen, jugular fossa, carotid canal orifice, hypoglossal nerve canal, occipital condyle, foramen ovale, foramen spinosus, etc. The shortest distance of sphenoid spine and the angle between these two points and these pores are described in Table 1.3. Anatomy of the Glomus jugular fossa: measurement of the longest diameter based on coronal plane and sagittal plane, left side is 14.2 鹵2.1 mm-1 mm 7.3 鹵1.6 mm, The right side was 15.5 鹵2.0mm and 9.7 鹵2.0mm. The results of chi-square test showed that the size of the jugular fossa was different between the right and left sides. 4 the shortest distance between the caudomastoid foramen and the jugular fossa was left.
