【摘要】：頭痛是臨床常見的癥候之一,分類繁多,病因尤為復(fù)雜。胸鎖乳突肌肌筋膜扳機(jī)點(diǎn)的存在是頭痛一個(gè)重要的因素。但與人們所認(rèn)識的頸源性頭痛不同的是肌筋膜性疼痛不累及頸椎的骨性結(jié)構(gòu)及頸椎間盤,只是與頸部的肌肉和筋膜有關(guān)。因此,對頭痛者,不應(yīng)忽略對枕頸部肌肉,特別是胸鎖乳突肌肌筋膜扳機(jī)點(diǎn)的詳細(xì)檢查。 第一部分 胸鎖乳突肌筋膜扳機(jī)點(diǎn)疼痛流行病學(xué)研究 材料和方法 選擇37名健康中老年人為研究對象,年齡為50—84歲,平均年齡為64歲。選擇35例健康在校大學(xué)生為對照組,年齡為19—30歲,平均年齡23歲。按胸鎖乳突肌在乳突部附著的情況,選擇乳突前、乳突中和乳突后三個(gè)常見的扳機(jī)點(diǎn)部位。通過壓力傳感器的監(jiān)測,并用VAS疼痛量化表進(jìn)行評分。 結(jié)果 老年男性的VAS疼痛評分(5.226±0.331)與老年女性的(5.531±0.379)的評分結(jié)果無顯著性差異(p0.05);青年男性的VAS疼痛評分(2.940±0.331)與青年女性的(3.982±0.405)的評分結(jié)果無顯著性差異(p0.05)。 中老年組壓痛評分為5.358±2.309,青年組壓痛評分為3.357±1.956。兩
[Abstract]:Headache is one of the common symptoms in clinic. The presence of the myofascial trigger of sternocleidomastoid muscle is an important factor in headache. But what is different from the known cervical headache is that myofascial pain does not involve the bony structure of the cervical vertebrae and the cervical intervertebral disc, but is only related to the cervical muscles and fascia. Therefore, the detailed examination of the occipitocervical muscle, especially the fascia trigger of sternocleidomastoid muscle, should not be neglected in patients with headache. Part I Epidemiological study on the pain of fascia trigger of sternocleidomastoid muscle Materials and methods A total of 37 healthy adults aged 50 to 84 years, with an average age of 64 years, were selected. 35 healthy college students were selected as control group, aged 19-30 years, with an average age of 23 years. According to the attachment of the sternocleidomastoid muscle in the mastoid part, three common trigger points were selected: before mastoid, mastoid and after mastoid. The pressure sensor was used to monitor and score with the VAS pain quantification table. Result There was no significant difference in VAS pain score between the aged male (5.226 鹵0.331) and the elderly female (5.531 鹵0.379), but there was no significant difference between the young male (2.940 鹵0.331) and the young female (3.982 鹵0.405).
